<br />Exhibit "A" <br />Program/Project Description and Specific Requirements <br /> <br />716(J~-'e--'DQ~~' <br /> <br />In consideration of the payments set forth in Exhibit liB", Contractor shall provide the <br />following services: <br /> <br />Provide vital safety net services to approximately 450 households, consisting of low-income <br />families, single parents, seniors, disabled persons and individuals seeking assistance in <br />meeting their basic human needs. To provide appropriate and immediate emergency <br />assistance including homeless assistance to families and single adults in the form of direct <br />service provision or referrals to other service providers. To prevent homelessness by <br />providing emergency and other services to families and individuals who are at risk of <br />becoming homeless. <br /> <br />1. Reporting <br />Contractor shall submit to the Department of Housing a Quarterly Performance Report within <br />30 days of the end of each quarter and shall consist of two parts: <br /> <br />./ Performance SummoN, to be submitted on form provided by the County <br />Department of Housing, which shall summarize the number of clients served by <br />their ethnicity and income. Data shall be entered for both households and <br />individuals. <br /> <br />./ Narrative Report which provides a narrative of highlights/details of both the <br />funded activity and of the Contractor's organization. A recommended format <br />will be provided by the County. The narrative is required even if no activity has <br />taken place within the Quarter. <br /> <br />An annual Performance Log will also be provided by County; this form provides the basic <br />demographic data to be maintained for each person and/or household served. <br />Contractor is not required to submit this form, but must maintain the information in a format <br />or data base that can readily be accessed at the time of anyon-site monitoring performed <br />by County staff. Contractor shall maintain files in their offices which shall contain the <br />information required in the Performance Reports. Household income shall be documented <br />by a statement of income signed by the client or verified by pay stubs, income tax returns, <br />report of benefits, pensions or other suitable verification of income. <br /> <br />All households or individuals served must have household incomes which do not exceed the <br />low income limits established by the Department of Housing & Urban Development. The <br />limits in effect for the current contract period will be provided to you by the County. <br /> <br />A t the end of the fiscal year Contractor shall submit a Fundinq Sources Final Report that <br />shows the total of all funds expended for the program.
