Description must include <br />(a) program specific objectives; (b) target population; (c) geographic impact (d) Description of <br />exact scope of services for which you plan to use funds. <br /> <br />The Fair Oaks Community Center (FOCC) Information and Referral Program provides vital safety net <br />services to low-income families, single parents, seniors, disabled persons, and individuals seeking <br />assistance in meeting their basic human needs. While the program's service area includes Redwood <br />City, Woodside, Atherton, and Portola Valley, approximately 25% of the clients served during the <br />current fiscal year by this program are from the unincorporated county jurisdiction of North Fair Oaks. <br />The program objectives are: 1) to provide appropriate and immediate emergency assistance <br />including homeless assistance to families and single adults in the form of direct service provision or <br />referrals to other service providers; 2) to prevent homelessness through the provision of emergency <br />and other services to families and individuals who are at risk of becoming homeless; and 3} to assist <br />families and individuals in their efforts to maintain their self-sufficiency, become self-sufficient or <br />regain their self-sufficiency. <br /> <br />Funds are requested to support services provided to unincorporated North Fair Oaks residents. <br /> <br />2. Identification of Priorities. How did you determine the needs for your Program/Project? Identify <br />the specific NOFA Objectives that your program/project meets. <br /> <br />The FOCC Information and Referral Program was initially created in response to a need identified <br />by the Human Services Agency (HSA) to provide complimentary service to the county's public <br />benefit program - providing supplemental services for public benefit recipients and providing <br />needed services to those not qualifying for public benefits. The program has evolved as HSA has <br />evolved - supporting the agency's efforts to assist families and individuals in regaining self- <br />sufficiency, working towards ending homeless in our community, directing families and individuals <br />to the available county services, and providing needed services for the working poor. Client <br />demand for services continues to demonstrate the need for this program. Over the last three <br />years, FOCC has increased it's focus on the coordination and provision of homeless services for <br />single adults and families as a part of the countywide HOPE Plan to End Homeless in San Mateo <br />County. In addition to coordinating regularly with HSA and other homeless services and safety <br />net providers, Fair Oaks participates regularly in various agency forums/processes including the <br />Core Service Agency Network, the Homeless Continuum of Care, the Homeless Services Provider's <br />Group, the Family Self-Sufficiency Team (FSST), the HOPE Lead Person's Collaborative, and <br />Redwood City Human Services Agency Network. FOCC was also integrally involved in the <br />development of the County's Homeless Prevention and Rapid Re-Housing Program (HPRP) - <br />enhancing our capacity to provide homeless prevention services to families and individuals in our <br />community . <br /> <br />The FOCC Information and Referral Program meets priorities B-2 and B-3. The safety net services we <br />provide respond to the identified needs by providing a Monday-Friday Drop-In Program - including a <br />combination of direct services and referrals for food, emergency shelter, transportation, housing- <br />related assistance, energy assistance, crisis intervention, legal assistance, and advocacy. <br />Approximately 25% of the clients served by this program are from North Fair Oaks. Services like our <br />monthly Family Harvest Food Program (provided in collaboration with the Second Harvest Food <br />Bank), shelter referrals and our rental/deposit assistance - including the recently launched HPRP <br />program - are aimed at helping to keep people in their homes inspite of the high cost of living in San <br />Mateo County. <br /> <br />NOFA FY 2010-11 Rehabilitation Program Application <br />
