Describe your overall organizational mission. <br /> <br />The mission statement of the Fair Oaks Community Center Information and Referral Program is: liThe <br />Human Services Unit of the Fair Oaks Community Center is a team of compassionate and committed <br />individuals that supports, educates, empowers, and serves as a bridge to resources for families and <br />individuals in the communities we serve. In addition, the team serves as a voice on behalf of the <br />community as a whole - identifying needs/issues of concerns and finding ways to address these <br />challenges." <br /> <br />4. Outreach. Describe your program's outreach efforts to ensure that the services are made known to <br />all persons of need. <br /> <br />Outreach for the Information and Referral Program is carried out through a variety of ways: <br /> <br />- As one of the seven Core Service Agencies, the Fair Oaks Community Center's services are <br />advertised through the distribution of flyers about the Core Services Network throughout the county- <br />primarily through the various offices of the Human Services Agency and other service providers <br />throughout the county. <br />- FOCC participates in community fairs/festivals throughout the year. <br />- FOCC's services are advertised in the Redwood City Activity Guide distributed to all Redwood City <br />households four times a year. <br />- Services are regularly promoted at the Redwood City quarterly human service agency network <br />meetings; with ongoing social service partners including the st. Vincent de Paul Homeless <br />Conference and other emergency services providers, through flyers distributed through the local <br />schools, and by our very own clients and partner agencies -as they refer others to our services. <br />- Additionally, FaCe is working with the HPRP collaborative to develop an additional outreach <br />strategy to raise awareness about homeless prevention services available to the community. <br />5. Collaboration. Describe how your agency collaborates with other service providers to meet the <br />various needs of your clientele. <br /> <br />Face collaborates closely with a variety of service providers to meet the various needs of both our <br />clients and the clients of the other service providers. One example of this is with our Early Entry Shelter <br />Program for single homeless adults. In working with the individuals in this program, we coordinate <br />with a number of partners to help meet the client's needs: <br /> <br />"'the Redwood City Sf.
