<br />to any location where records pertaining to the Program may be found. Partner <br />Organization shall comply with any reasonable request made by Public Allies San <br />Franciscio/Silicon Valley or Public Allies, Inc. for additional information, clarification or <br />documentation on the Program, whether in connection with any evaluation or reports, <br />monitoring visits, or otherwise. Partner Organization shall at all times maintain <br />appropriate records pertaining to the Program at a location that will afford Public Allies <br />San Francisco/Silicon Valley or Public Allies, Inc. reasonable access thereto during a <br />monitoring visit. <br /> <br />F. Evaluation. Partner Organization shall participate as necessary <br />with all Public Allies San Francisco/Silicon Valley or Public Allies, Inc. evaluation <br />programs and projects. This includes, but is not limited to, defining and gathering <br />specific data required, including reporting procedures, quality control, analysis and <br />interpretation, and other elements necessary and appropriate for the evaluation of the <br />Program. Partner Organization will evaluate each Ally's individual performance at the <br />end of each program time for a complete and programmatic evaluation of the Program. <br /> <br />G. Other ReportinQ and Notification. Partner Organization will provide <br />Public Allies San Francisco/Silicon Valley or Public Allies, Inc., upon its request, with <br />time logs or other relevant data, regarding the performance of the Ally or Allies placed <br />with Partner Organization, as Public Allies San Francisco/Silicon Valley or Public Allies, <br />Inc. may need to gather on the achievement of national direct performance <br />measurements under the Program. In addition, Partner Organization will promptly notify <br />Public Allies San Francisco/Silicon Valley if an Ally placed with it exhibits erratic <br />attendance or otherwise stops or quits reporting to it. <br /> <br />H. Worker's Compensation, Liabilitv Insurance and Unemplovment <br />Insurance. Public Allies San Francisco/Silicon Valley shall provide worker's <br />compensation insurance and pay unemployment insurance and Social Security taxes to <br />the full extent required by law and accepts full responsibility for the payment of <br />premiums for worker's compensation insurance, and unemployment insurance and <br />Social Security taxes, as well as all income taxes required by law, for any and all Allies <br />who are performing services for Partner Organization, in connection with the Program <br />as long as the Ally is on the Public Allies San Francisco/Silicon Valley's payroll. In <br />addition, in those states where Public Allies is not required to provide worker's <br />compensation insurance coverage for an Ally, Public Allies San Francisco/Silicon Valley <br />will ensure that the Ally is covered by its general liability insurance coverage, which <br />includes accidental death and dismemberment coverage. <br /> <br />I. Nondiscrimination. In carrying out its part of the Program, Partner <br />Organization shall not discriminate against any Ally or Program applicant because of <br />race, color, religion, sex, sexual orientation, national origin, disability or any other <br />characteristic protected by local, State or Federal law. Partner Organization shall take <br />affirmative action to ensure that applicants are treated during the Program without <br />regard to their race, color, religion, sex, national origin, sexual orientation, disability or <br />any other characteristic protected by local, State or Federal law and eliminate the <br />effects of past discrimination.
