<br />DUPLICATr <br /> <br />ORIGINAL <br /> <br />MEMORANDUM OF UNDERSTANDING <br /> <br />BE1WEEN <br /> <br />Samaritan House and City of Redwood City/Fair Oaks Community Center <br /> <br />FOR ASSISTANCE WITH HOMELESSNESS PREVENTION AND RAPID RE- <br />HOUSING (HPRP) SERVICES IN THE CITIES OF SOUTH SAN FRANCISCO, <br />REDWOOD CITY AND SAN MATEO <br /> <br />Samaritan House_and City of Redwood City/Fair Oaks Community Center agree to <br />collaborate for the purpose of implementing the Cities of South San Francisco, Redwood City <br />and San Mateo HPRP (HPRP), a homeless prevention and rapid re-housing program designed <br />to bolster housing stabilization and provide temporary financial assistance and/or services to <br />help persons gain housing stability in the cities of San Mateo, Redwood City and South San <br />Francisco. This agreement is wholly dependent on funding from the State of California Housing <br />and Community Development (HCD) Federal Stimulus Funds under the American Recovery and <br />Reinvestment Act of 2009 (ARRA). Any shortfall from the State funding will result in a corollary <br />shortfall in payment from Samaritan House to City of Redwood City/Fair Oaks Community <br />Center. City of Redwood City/Fair Oaks Community Center will hereinafter be called <br />"Contractor" for this agreement. <br /> <br />Scope of Services to be provided <br /> <br />CONTRACTOR agrees to provide: <br /> <br />1) Homeless Prevention Services <br />a) Financial Assistance <br />b) Housing Stabilization and Relocation Services <br /> <br />All service activities to be in accordance with Exhibit A - Scope of Services. <br /> <br />Contractor will hold primary responsibility for the design and implementation of its programs <br />and will provide Samaritan House with any updates concerning changes in service availability. <br />As needed.1 based on future program evaluation findings and/or funding availability, the scope <br />and design of services may be modified by joint agreement between Samaritan House and <br />Contractor. If Contractor fails to provide services as scheduled or if service provision is <br />significantly delayed or interrupted, Samaritan House may terminate this agreement with a <br />minimum of 30 days notice to Contractor. <br /> <br />All services will be provided as a component of the larger HPRP collaborative, for which the <br />program oversight committee (described below) will collaborate on coordination of services, <br />resources and objectives. If, in collaboration with the oversight committee, Samaritan House <br />determines in the future that Contractor's services no longer meet program objectives, or if <br />funding availability dictates that these services must be reduced or eliminated, this agreement <br />may be terminated by Samaritan House with a minimum of 30 days notice to Contractor. <br /> <br />Individualized Housing and Service Plan documents <br /> <br />Contractor agrees to use and complete all forms and requested data collection required by <br />Samaritan House and HCD related to this project.
