<br />includes, but not limited to, the following: Initial Screening Questionnaire, Risk Determination <br />Assessment, Client Self Declaration of Housing Status, Client Self Declaration of Income, Verification <br />of Income, Homeless Certification, HPRP Staff Affidavit Form, HPRP Client Intake Form, HMIS Data <br />Collection Form, Client Assistance Agreement, Budget and Housing Forms, HPRP Client Exit Form, <br />HMIS Discharge Form, 3 & 6 Month Follow-up form, and other forms as required. Additional forms and <br />documents may be added as needed during the grant period. <br /> <br />Note: Required dataformsfor financial assistance are to be completed and submitted to <br />Samaritan Housefor each client PRIOR to issuing financial assistance checks to vendors. <br />Incomplete or inaccurate dataforms will be returnedfor correction before assistance checks <br />will be processed. <br /> <br />Follow up interviews and/or surveys will be conducted on program participants at 3 and 6 <br />months. Samaritan House and Contractor agree to coordinate efforts on the implementation of <br />the follow-up component. <br /> <br />Integration of Services & Interagency Communication <br /> <br />Samaritan House and Contractor will work toward the highest degree of service integration and <br />interagency communication. Specific procedures to be followed will include: <br /> <br />Training: Contractor will participate in HPRP trainings and meetings for partner and <br />agencies to be held by Samaritan House and the County of San Mateo concerning <br />HMIS data collection and other program procedures. <br /> <br />Program oversight committee: to ensure all partners have opportunities to participate in <br />the oversight of the collaborative, an oversight committee comprised of one <br />representative from each partner agency will meet quarterly to monitor the success of <br />program logistics, interagency communication, service coordination, service/program <br />modification, and program outputs and outcomes. Committee will review significant <br />staffing changes that may affect service provision. <br /> <br />Data collection and reporting: Both agencies agree to all data collection and HMIS <br />requirements as outlined in the proposal and grant agreement and required by HUD <br />regulations and HCD. Samaritan House will have primary responsibility for entering <br />such data into the HMIS and reporting on collected data to HCD. <br /> <br />Outreach and Marketing: Work collaboratively with the oversight committee to <br />implement outreach and marketing efforts to eligible participants for HPRP funding. <br />Conduct outreach and marketing activities for HPRP funding within contractor's <br />scope of services. <br /> <br />Insurance <br /> <br />Both Samaritan House and Contractor will maintain adequate liability and property insurance. <br />Each agency will name the other as an additional insured on their policies. <br /> <br />Personnel, Space, and Materials <br /> <br />Contractor will be responsible for recruitment, hiring, supervision, training, payroll, and <br />benefits of their staff providing the services as described above under Services to be provided. <br />Time and activity reports are required for all reimbursement of personnel costs related to this <br />agreement.
