Plan Name: Premium Contribution Plan <br />Funding: Funded through contributions by the City and through elective pre -tax <br />contributions from regular salary and/or wages. Administration fees <br />funded by the City's general operating funds. <br />Maximum: None <br />2. Plan Name: Medical Expense Reimbursement Plan <br />Funding: Funded through contributions by the City and through elective pre -tax <br />contributions from regular salary and/or wages. Administration fees <br />funded by the City's general operating funds. <br />Maximum: $666.67 a month ($8,000 each Plan Year) <br />Plan Name: Dependent Care Assistance Plan <br />Funding: Funded through contributions by the City and through elective pre -tax <br />contributions from regular salary and/or wages.
