Additional forms and <br /> documents may be added as needed during the grant period. <br /> Note: Required data forr�ns for financial assistance are to be compieted and submitted to <br /> Samaritan House for each client PRIOR to issuing financial assistance checks ro uendors. <br /> Incomplete or inaccurate data forms wiIl be returned for correction before assistance checks <br /> will be processed. <br /> Follow up interviews and/or surveys will be conducted on program participants at 3 and 6 <br /> months. Samaritan House and Contractor agree to coordinate efforts on the implementation of <br /> the follow-up component. <br /> Integration of Services & Interagency Communication <br /> Samaritan House and Contractor will work toward the highest degree of service integration and <br /> interagency communication. Specific procedures to be followed will include: <br /> Trainin� Contractor will participate in HPRP trainings and meetings for partner and <br /> agencies to be held by Samaritan House and the County of San Mateo concerning <br /> HMIS data collection and other program procedures. <br /> rogram Qversight committee: to ensure all partners have opportunities to participate in <br /> the oversight of the collaborative, an oversight committee comprised of one <br /> representative from each partner agency wil] meet quarterly to monitor the success of <br /> program logistics, interagency communication, service coordination, service/program <br /> modification, and program outputs and outcomes. Committee will review significant <br /> staffing changes that may affect service provision. <br /> D�ollection and reportin�: Both agencies agree to all data collection and HMIS <br /> requirements as outlined in the proposal and grant agreement and required by HUD <br /> regulations and HCD. Samaritan House will have primary responsibility for entering <br /> such data into the HMIS and reporting on collected data to HCD. <br /> Outreach and Marketing: Work collaboratively with the oversight committee to <br /> implement outreach and marketing efforts to eligible participants for HPRP funding. <br /> Conduct outreach and marketing activities for HPRP funding within contractor's <br /> scope of services. <br /> Insurance <br /> Both Sarnaritan House and Contractor will maintain adequate liability and property insurance. <br /> Each agency will name the other as an addidonal insured on their policies. <br /> Personnel, Space, and Materials <br /> Contractor will be responsible for recruitment, hiring, supervision, training, payroll, and <br /> benefits of their staff providing the services as described above under Services to be provided. <br /> Time and activity reports are required for all reimbursement of personnel costs related to this <br /> agreement.
