Our estimated level of effort for this work is included as
Attachment B.

This additional work is necessary because of the following reasons:

.

Caltrans request to the City to revise the profiles for the diagonal on ramp to northbound
Route 101 and the Island Parkway direct connection so that the on ramp stays at-grade
and Island Parkway is elevated.

Our original scope of work included the design of one bridge, the diagonal on ramp
bridge structure over Island Parkway direct connection- A second bridge, spanning the
existing Belmont Channel box culvert, was added when it was discovered during the
geotechnical investigation that there was an underlying layer of new bay mud on the east
side of the fi-eeway. The current revised scope of work includes only the Island Parkway
Overcrossing structure. In addition, a parapet wall must be added to the existing box
culvert to accommodate utility relocations.

Our original scope of work included only utility relocation coordination as it was
assumed that the respective utility owners would be responsible for the design of their
facilities that may be required to be relocated. The revised profiles for the on ramp and
Island Parkway require the relocation of the sanitary sewer force main and water main
because of the embankment construction. Therefore, We have included the design of the
sanitary sewer force main in our revised scope of work. However, the design of the water
main relocation is assumed to be the responsibility of Mid-Peninsula Water and has not
been added to our scope of work at this time.

Our original scope of work did not include preparation of any permits other than Caltrans
Standard Encroachment Pennit for construction. The impacts to Army Corps
jurisdictional wetlands have necessitated the need to apply for both an Army Corps
Nationwide Permit and a permit from the Regional Water Quality Control Board. At the
city's request, we have included these tasks in our revised scope of work. In addition,
our subconsultant, HT. Harvey was requested by the city to prepare the Mitigation and
Monitoring Plan for wetland impacts.

CUPERTINO

PlEASANTON

REDWOODCllY S.\CRAMENTO
www,markthomas.com

SAUNAS

SAN JOSE

WALNUT CREEK
