• <br /> ATTACHMENT K <br /> CONTRACTOR'S COMPENSATION AND RATE SETTING PROCESS <br /> 4. Application for Contractor's Compensation Adjustment <br /> 4.1 Preparation of Application for Contractor's Compensation Adjustment <br /> Contractor is required to submit an Application for Contractor's Compensation Adjustment <br /> (Application) annually by July 1 for determination of Contractor's Compensation for the <br /> following Rate Year. This Section describes the content of the Application. <br /> A. Reporting of Operational Information <br /> Operational information shall be reported in total as well as disaggregated by each <br /> Line of Business, by Service Sector and by Member Agency. The information shall <br /> be submitted on forms provided by Contractor and approved by SBWMA. This data <br /> is required to allocate Contractor's Compensation to each Member Agency. <br /> Operational information to be provided includes, but is not limited to: <br /> • Tonnage collected by Line of Business; <br /> • Number of accounts by Line of Business and account type (i.e., Container size, <br /> Collection frequency, and material type); <br /> • Number of pulls for Roil -Off service; <br /> • Number of lifts for Bin service; <br /> • Number of Containers in service by Line of Business; <br /> • Set -out rates by Line of Business; <br /> • Number of routes and annual route hours by Line of Business; <br /> • Number and type of vehicles by Line of Business; <br /> • Annual route labor hours by Line of Business; and <br /> • Roster of all personnel by category including direct, indirect, and general and <br /> administrative (G &A). <br /> B. Audited Financial Statements <br /> Financial statements (balance sheet, income and expense statement) for the <br /> operations covered by this Agreement for Contractor's immediately preceding fiscal <br /> year (October 1 — September 30) shall be submitted with the Application. The <br /> financial statements shall be accompanied by a report of an independent Certified <br /> Public Accountant licensed by the California Board of Public Accountancy stating <br /> that (i) it has audited the financial statements in accordance with auditing standards <br /> generally accepted in the United States, and (ii) in its opinion the financial statements <br /> present fairly, in all material respects, the financial position of Contractor as of <br /> September 30 of the year under review and of the immediately preceding year, and <br /> the changes in its financial position for the years then ended in conformity with <br /> United States generally accepted accounting principles. <br /> The Certified Public Accountant's report shall also contain a separate statement <br /> identifying the amounts of audited revenue and expense that are attributable to the <br /> last nine months of the Contractor's fiscal year (January - September 30). <br /> In addition, Contractor shall submit financial statements covering the last three <br /> months of the preceding calendar year (October 1 - December 30) and a compiled <br /> Collection Services Franchise Agreement with Recology San Mateo County Attachment K <br /> City of Redwood City Contractor's Compensation and Rate Setting Process <br /> Page 19 of 26 <br />
