The actual maximum monthly contribution per active`'" <br />worker, once these charges have been deducted, is estimated to be $ T - . 2C 1 1'5 -1 <br />.e' 14 r9.3�t�6 <br />22.11-71n the event the Kaiser Family premium increases above $t2ga.tt during the <br />term of this MOU, the City will increase its contribution in an amount equal to such an <br />increase. <br />Effective January 1, 2012, City contribution to the cafeteria plan will be: <br />• A maximum of ninety -five percent (95 %) of actual cost of coveraae for dental <br />and vision coverage, minus <br />• The PEMHCA minimum monthly employer contribution. <br />22.1.8 For Library workers covered by this Memorandum of Understanding, this amount <br />shall be prorated for permanent part-time workers working nineteen (19) or more <br />hours per week. <br />For all other workers covered by this Memorandum of Understanding, this <br />amount shall be prorated for each permanent part-time worker working twenty <br />(20) or more hours per week. <br />22.1.9 Until December 31 2011, a Any funds remaining at the end of the fiscal year in <br />each individual Cafeteria Plan account shall be returned to the general fund for use <br />consistent with IRS Code Section 125. Effective January 1. 2012, the City will <br />discontinue contributions to flexible spending accounts on behalf of workers. <br />22.1.10 December 31 2011 i ff a worker elects no health plan coverage and <br />provides evidence of other coverage during annual open enrollment, as a health <br />savings /cash option, the City and the worker will share equally the amount <br />designated in Section 22.1.7/22.1.8 above as the City's actual maximum monthly <br />to workers for opting out of medical coverage. <br />22.2 Dental Insurance <br />The City shall continue to provide to eligible workers and dependents fully paid <br />dental insurance through the Delta Dental Plan effective October 1, 2003 <br />through the term of this agreement to include coverage's as follows: $2,000.00 <br />annual cap for basic coverage and $1- 2,000.00 lifetime cap for orthodonture, <br />provided however, that upon the request of the City, the parties will re -open this <br />Article 22.2 to meet and confer on modifying 22.2 to provide a different dental <br />insurance plan.
