<br /> 6.1G <br /> Page 9 <br /> 1) Mutual clients (HSA and Contractor's) and those who consent will have <br /> basic intake data entered into SMART. Contractor will enter data <br /> identifying contractor as a current service provider for the client. <br /> 2} All other clients will have the required SMART entries entered into <br /> SMART (name, date of birth and sex). <br /> 3) Exceptions are undocumented clients unknown to HSA and those <br /> persons who do not consent to ha ve their data entered into SMART. <br /> 4) The Contractor agency has SMART functionality in the most current <br /> version. <br /> Contractor will explain to clients the benefits of having their demographic <br /> information entered into SMART and shared with HSA, and will encourage all <br /> clients to consent to have the information shared with HSA. <br /> E. Technoloav Contractor will work with HSA on an ongoing basis regarding <br /> SMART and other systems and technology issues. HSA will supply SMART <br /> connectivity to Contractor. and pay all monthly and annual on-line charges and <br /> Contractor shall maintain sufficient hardware capable of running SMART during the <br /> term of this Agreement. <br /> F. Release of information A signed client consent to services and release of <br /> information between the Contractor and the Human Services Agency will be <br /> obtained. Feedback on referrals will be timely. <br /> G. Evaluation HSA and Contractor, with the assistance of an HSA Planning and <br /> Evaluation Specialist, will develop appropriate measures to assess the outcome of <br /> services. <br /> - <br />
