PREVENTATIVE MAINTENANCE SERVICE AGREEMENT SCOPE A scheduled preventative maintenance agreement that will consist of a complete 21 point mechanical and electrical inspection of every pump /mixer listed per our contract. This inspection will be on a 12_ month basis followed by a complete report of the findings. The inspection is based upon agreement of both parties involved in the services and terms stated in detail below. Please note: This service quote does not include any overhaul or shop repairs. Sales Tax on parts not included. TERMS This agreement includes all labor and parts to complete the 21 point inspection listed on page 4. Any additional repairs not stated on the 21- point check list required during the inspection would be performed only by written authorization. 1'r Water & Wastewater Flygt U.S.A. Standard Terms & Conditions, Payment Requirements (Net 30 Days) pending credit approval by FRC, Standard Warranty applies. 11 "1` Water & Wastewater Flygt U.S.A. assumes no liability for loss of use or for any consequential damage with respect to the operation of the pumps, equipment or accessories: covered in this agreement. It is the customers responsibility to have the following at the job site when our service technician(s) arrive: A. Water supply for washing pump(s) yes no X__ B. 120 Volt electric supply yes X no C. Mechanic to assist in the inspection and learn proper techniques yes no D. Electrician. (if control not supplied by ITT Water& Wastewater yes no X E. Supply lifting equipment due to location and/or size of pump(s) yes no The fee for the schedule "A" " agreement is $15.124_10 for the complete 21 point inspection, which is valid for a _ 1_ - year period. ITT Water & Wastewater Five U.S.A. must receive, a purchase order prior to the work being perforaed. Any additional parts & labor described under. schedule ' 13" may require an additional purchase .order numberif the customer wishes to separate the costs between "A" & "13 ".
