Will attend partnership meetings and participate in information sharing, <br /> problem-solving, evaluation and data collection process. <br /> RCSD through the Hoover School will: <br /> 1. Provide 0.25 FTE Project Coordinator to coordinate project activities and <br /> staff, ensure that funds are spent appropriately, document project activities <br /> and accomplishments, and provide required grant reports. <br /> 2. Identify and refer students and families for services. <br /> 3. Coordinate and support SCVP Committee through Project Coordinator <br /> 4. Attend Partnership meetings and participate in information shazing, problem- <br /> solving, evaluation, and data collection process. <br /> 5. Provide adequate program and office space as requested by the agency. <br /> RCSD will: <br /> 1. Act as a Fiscal agent for the partnership. <br /> 2. Oversee and maintain contract agreements. <br /> 3. Identify and communicate training opportunities. <br /> 4. Oversee and support evaluation and data collection. <br /> 2. EQUIPMENT AND MATERIALS <br /> CONTRACTOR shall provide all equipment, materials, and supplies necessary <br /> for the performance on the agreement. <br /> 3. USE OF SUBCONTRTACTORS <br /> CONTRACTOR shall not assign this agreement or any portion thereof to a third <br /> party without the prior written consent of RCSD. <br /> 4. LICENSES AND PERMITS <br /> CONTRACTOR shall obtain and keep in force all licenses, permits, and <br /> certificates necessary for the performance of this agreement. <br /> 5. INSURANCE <br /> CONTRACTOR understands the RCSD does not provide liabiliTy, malpractice, or <br /> workers compensation insurance for the CONTRACTOR. The CONTRACTOR <br /> is responsible fort the maintenance of personal and professional insurance to <br /> provide protection far any claims arising out of service under this agreement. The <br /> CONTRACTOR assumes all liability for services provided under this contract. <br /> The CONTRACTOR will provide proof of insurance and will identify RCSD as <br /> an additional insured. <br />
