TENTATIVE AGREEMENT BETWEEN THE CITY OF REDWOOD CITY <br /> AND REDWOOD CITY POLICE SERGEANTS ASSOCIATION (PSA) <br /> Effective Januarv 1. 2012. Citv contributions to the Cafeteria Plan will be limited <br /> to contributions described in Section 10.2.2 and will not include credit to a <br /> flexible soendinp account. <br /> I 10.32 Dental <br /> The City shall continue to provide to eligible employees and dependents fully paid dental <br /> insurance through Delta Dental Fee for Service program <br /> agreeme�� or upon the establishment of a mutually agreed upon alternative program, to <br /> include coverage's as follows: $2�,000 annual cap for basic coverage and $2,5000 <br /> lifetime cap for orthodonture. , <br /> Effective Januarv 1. 2012. the Citv will oav ninetv oercent (90%) of the dental insurance <br /> premium for eliqible emolovees end deoendents. <br /> I 10.4�3 Life Insurance <br /> Effective Seotember 1. 2011, t�he City shall c�eati�e—te--e#eF ro ovide to eligible <br /> employees #I�life insurance coveraae of $50.000. , <br /> �4. I' <br /> I 10.76 Vision <br /> Effective with the first full pay pe�iod on or after September 1, 2003, the City will contract <br /> with Vision Service Plan (VSP) o� comparable vision care provider to provide vision care <br /> benefits for employees and their dependents. The Vision Service Plan "B" provides for <br /> an exam every 12 months, lenses every 12 months if needed, and frames every 2 years, <br /> if needed.
