serve as a juror shall not have hislher regular starting or quitting time changed as a result of being called for <br /> jury service. <br /> �&516.6Matemity Leave of Absence Without Pay <br /> 48:5:416_6.1 Maternity leave of absence without pay or benefits shall be granted upon request to non- <br /> disabled probationary and regular female employees for that period of time requested up to one <br /> (1) year. <br /> �&4216_6.2Maternity leave shall be granted when the following conditions have been met: <br /> a816.6.2.7 The employee shall notify her immediate supervisor in writing accompanied by <br /> her physician's certificate of pregnancy as soon as possible after pregnancy has <br /> definitely been detertnined, but no later than ninety (90) days prior to the <br /> tentative date on which the leave is to begin. Such notice shall include the <br /> tentative dates on which the leave shall begin and end. <br /> 4816.6.2.2 Within thirty (30) days of the beginning of the matemity leave, the employee shail <br /> submit to the Human Resources Director the specific date she intends to begin <br /> the leave, accompanied by her physician's written statement attesting to the <br /> employee's ability to continue performing the full schedule of her duties and <br /> responsibilities. She shall continue on active duty until the specific date <br /> providing she performs the full duties and responsibilities of her position and <br /> fumishes additional health statements from her physician upon reasonable <br /> request. <br /> �76.6.2.3 Prior to the establishment of a specific date for retum to duty, the employee shall <br /> submit to the Human Resources Director a notice of intention to return to duty, <br /> accompanied by her physician's statement certifying that the employee is <br /> medically qualified to assume regular duties and responsibilities. <br /> 4816.6.2.4 The Human Resources Director or hismer designee may designate the specifc <br /> beginning and ending dates within the pay period requested by the employee to <br /> meet the needs of the employee and the City. <br /> 3816.6.3 The employee on leave shall be retumed to her original posiiion or if ihat position is not in <br /> existence, to an equivalent position within her class�cation. <br /> 4876.6.4 A matemity leave, absent physical disability, is granted without pay for the duration of leave. The <br /> employee may elect to continue medical and dental insurance coverage for up to one (1) year <br /> during this leave at her own expense. <br /> �76.6.5 An employee may coordinate a portion of her matemity leave with provisions of the Family and <br /> Medical Leave Act, as described in Section �:516.9. ' <br /> 4816.7 Leave for Pregnancy Disability <br /> 4816.7.1 . Employees who are working are entitled to use personal illness and injury leave for disabilities <br /> caused or contributed to by pregnancy, miscarriage, childbirth, and recovery there from on the <br /> same terms and conditions goveming leaves of absence for other iliness or medical disability. <br /> Such leave shall not be used for child care, child rearing, or preparation for childbearing, but shall <br /> za <br />
