EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Middlefield Road Underground Utility District 2011-1 Project <br /> Design Scope of Work <br /> PROJECT DESCRIPTION: <br /> Consultant shall provide design and engineering services for the Middlefield Road Underground <br /> Utilities District 2011-1 Projed. The objectives of the Middlefield Road Underground Utilities District <br /> 2011-1 Project are to remove existing overhead utility structures; install facilities for supplying electric, <br /> communication, and similar services underground; and make streetscape improvements that increase <br /> pedestrian and bicyclisYs mobility. The project area is defined as the length of Middlefield Road <br /> between Main Street and MacArthur Avenue. <br /> For the first phase of this agreement, the Consuitant shall specifically provide design and engineering <br /> services for this project to include aerial topography, utility surveying, legal description, and production <br /> of the Joint Trench Intent Drawing. The Joint Trench Intent Drawing shall show the City's preferred <br /> proposed utility trench alignment and the stree[ layout. 7he affected utility companies' (PG&E, AT&T, <br /> Comcast, Astound) will use these Joint Trench Intent drawings to prepare their individual utility design <br /> for their underground facilities. eased on discussions with PG&E, their design will take up to two years <br /> to complete (Summer 2014). <br /> Upon completion of the utility companies design for their facilities, to the City may amend this <br /> agreement to include finalizing the composite joint trench drawings, preparing "Form B", finalizing the <br /> streetscape design, and construction support services. <br /> DESIGN: <br /> TASK 1—AERIAL TOPOGRAPHY <br /> Consultant shali: <br /> 1. Set flight crosses for aerial topography using horizontal and vertical controls tied to City's basis <br /> of bearing and bench. <br /> Deliverable: <br /> a. 1"=40'scale map of city right-of-way for district and streetscape improvements shall be <br /> submitted within five (5) weeks from Notice to Proceed (NTP). <br /> TASK 2—UNDERGROUND UTILITY LOCATING AND MAPPING <br /> Consultant shall: <br /> 1. PerForm field survey to identify detectable subsurface interferences where project planning <br /> and construction activities are proposed. <br /> 2. Probe accessible storm drains and sanitary sewer lines. <br /> 3. Investigate proposed boring locations and subsurface obstructions. <br /> 4. Surface trace detected electrical lines with red spray paint. <br /> 5.
