EXHIBIT "A" <br /> SCOPE OF SERVICES AND FEE <br /> PROJECT DESCRIPTION: <br /> Consultant shall provide the design and engineering services for the Middlefield Road Underground <br /> Utilities District 2011-1 Project. Per the original agreement dated December 20, 2011, Bellecci prepared <br /> the joint intent drawings which were submitted to the various utility companies in July 2012. On August <br /> 26, 2013, a public hearing was held regarding the addition and expansion of the utility district territory. <br /> Minor adjustments to the boundary were required by PG&E.The consultant provided additional services <br /> for the revisions to the adopted boundary map and legal description, and for congruency, to the joint <br /> trench intent drawings that was resubmitted to the utility companies in September 2013. Additional <br /> work is required for the coordination with the utility companies during the review phase of the intent <br /> drawings which includes monthly meetings where utility companies will update the City of their <br /> progress. <br /> The undergrounding project presents an opportunity for streetscape improvements. The modifications <br /> to the joint trench intent drawings affect and inform the preliminary streetscape layout alternatives. <br /> Additional information about the right-of-way is also required to determine the feasibility of the <br /> streetscape alternatives which are to be presented to the community in future study sessions and <br /> community outreach meetings. <br /> The scope of the agreement is expanded to include the following: <br /> ADDITIONAL SERVICES: <br /> (Task 1-4 of original contract are completed) <br /> AMEND TASK 4—ADDITIONAL MEETINGS WITH CITY,COUNTY AND UTILITY COMPANIES <br /> Consultant will attend meetings at City's discretion for utility coordination or other related joint trench <br /> meeting. Three meetings are completed,with 2 meetings remaining in the budget. <br /> TASK 5—REVISE JOINT TRENCH INTENT DRAWINGS TO CITY AND UTILITY COMPANIES <br /> Consultant shall revise joint trench intent drawings to reflect the adopted revised boundary map dated <br /> July 11, 2013. The changes to the Joint Trench Intent Drawing are adjusted to fit the topographic aerial <br /> topo, but not on the Block Maps which are on a different coordinate base. There are three changes to <br /> the Block Maps which have been incorporated into the Joint Trench Intent drawing. <br /> Deliverable: <br /> a. Submit updated joint trench drawings. <br /> b. Submit an AutoCAD file and a PDF of the revised intent drawings to the City and PG&E. <br /> c.
