The Port will make reasonable attempts to contact references. If a reference cannot be contacted, no points will be awazded for that reference. 10.4 Essential Requirements for Contractor Qualification - Pass / Fail The essential requirements for Contractor qualification are attached to this RFQ as Appendix A. The Contractor will be immediately disqualified if the answer to any of quesrions 1 through 5 and 10 through 11 is "no". The Contractor will be immediately disqualified if the answer to any of questions 6 through 9 and 12 is "yes". If the answer to question 8 is "yes," and if debannent would be the sole reason for denial of pre-qualification, any pre-qualification issued will exclude the debannent period. 10.5 Business History, Performance, Safety, and Workers Comp - 25 points Additional, scorable questions aze attached to this RFQ as Appendiac B. These questions cover business history, organizational performance, compliance with occupational safety and health laws, workers' compensarion, and other labor legislation. The scoring for each question follows the question. There are a total of 129 available points in this section. To arrive at the point total to be used in the RFQ, the awazded points from Appendix B will be pro-rated. 10.6 RFP Selection Criteria The qualification score based upon this RFQ will carry over to the RFP selection criteria. Final selection will be based upon the following weighted measures: • RFQ Qualification Score - 25% • Technical Approach - 25% • Cost - 50% - The low bid will be awazded 50 points. Poinu will be awazded for other bids according to the following formula: Poinu Awarded =(1 -(Proposed Bid - Low Bid)/(Low Bid)) * 50 Additional detail on how these scores will be calculated will be contained in the RFP. 11 Format of Qualification Statements The cover letter, executive summary, qualifications and experience, and references portion of the RFQ response should be limited to 30 single sided pages or 15 double sided pages. Responses to Appendix A, B, C, and D do not count towazds the page total. Five copies of the qualification statement must be delivered to the Port by 5:00 pm on Friday, Mazch 16, 2012.
