V <br /> � ORIGIN�L <br /> AMENDMENT N0.1 TO AGREEMENT FOR CONSULTING SERVICES <br /> Nadine Levin <br /> �� <br /> This Amendment No.1 is entered into and effective as of the day of <br /> �� , 2012, amending the agreement dated April 1, 2012 (the <br /> "Agreement") by and between the City of Redwood City, a charter city and municipal <br /> corporation ("City"), and Nadine Levin, ("Consultant") (collectively, the "Parties") <br /> RECITALS <br /> A. On April 1, 2012 the Parties executed Agreement for Consulting Services <br /> Nadine Levin; and <br /> B. The Parties have negotiated and agreed to a supplemental scope of work, <br /> which is attached to and incorporated by this reference as Exhibit "A," Scope of <br /> Services as part of a grant being funded by the San Mateo County Office of Education- <br /> the Safe Routes to School (SR2S)Activities in the Redwood City School District. <br /> NOW, THEREFORE, in consideration of these recitals and the mutual covenants <br /> contained herein, the Parties agree as follows: <br /> 1. In addition to those services contained in the Agreement, as may have <br /> been amended from time to time, Consultant will provide those services described in <br /> Exhibit"A." <br /> 2. City will pay Consultant for atl work associated with those services <br /> described in Exhibit "A" Fifteen Thousand dollars ($15,000), for an aggregate amount of <br /> ($24,999). Consultant wilf provide City, on a monthty basis, copies of invoices <br /> sufficiently detailed to include hours performed, hourly rates, and related activities and <br /> costs for approval by City. <br /> 3. Consultant will complete all work described in Exhibit "A" by June 30, <br /> 2013. <br /> 4. All other provisions of the Agreement, as may have been amended from <br /> time to time, will remain in full force and effect. <br /> 5. All requisite insurance policies to be maintained by Consultant pursuant to <br /> the Agreement, as may have been amended from time to time, will include coverage for <br /> this Amendment. <br /> 6.
