6.1.1. - Page 12 <br /> PROPOSAL INFORMATION REQUIREMENTS <br /> 1. Mission of the organization and Key Activities <br /> The mission of the Fair Oaks Community Center (FOCC) is to create community by providing <br /> facilities, personnel, and programs necessary to support and coordinate human services for <br /> populations within Redwood City and North Fair Oaks —supporting, educating, empowering and <br /> serving as a bridge to resources for families and individuals in the community. Fair Oaks <br /> Community Center is a core service agency as designated by San Mateo County serving Redwood <br /> City, Unincorporated North Fair Oaks,Atherton, Woodside, and Portola Valley. <br /> 2. Project Description <br /> Funds are being sought to support vital food, shelter, and homelessness prevention services <br /> provided at the Fair Oaks Community Center to low-income families and individuals. The primary <br /> purpose of this collaborative project is to 1) provide appropriate and immediate food and homeless <br /> assistance;2) prevent homelessness through the provision of emergency, legal, and other services; 3) <br /> provide food for low income older adults who attend the Fair Oaks Adult Activity Center, and 4) <br /> assist families and individuals in their efforts to maintain their self-sufficiency, become self- <br /> sufficient, or regain their self-sufficiency. <br /> The three components/collaborative parmers for this project are: <br /> o Fair Oaks Community Center Information and Referral Program providing— <br /> o Emergency Food Assistance <br /> o Monthly Family Harvest Food Program <br /> o CalFresh benefits assistance <br /> o Referrals to other local food programs <br /> o Shelter Intake and Referrals for individuals and families <br /> o Early Entry Shelter Program for Single Homeless Adults <br /> o Homelessness Prevention through Housing Assistance <br /> o Case Management Services related to Food and Shelter <br /> o Information and Referrals for shelter and other homeless services <br /> o Legal Aid Society of San Mateo County providing— <br /> o Weekly Housing Clinic at Fair Oaks Community Center providing eviction defense to <br /> approximately 240 San Mateo County tenant households each year. <br /> o Advisory support to FOCC Human Services Specialists when legal issues related to <br /> housing arise for individual clients. <br /> o In-Service Training for FOCC Human Services Specialists and Peninsula Family Services <br /> Case Manager regarding relevant housing law to help staff assess clients' needs and risks <br /> with regards to becoming homeless. <br /> o Peninsula Family Service providing— <br /> o Breakfast and Lunch for low-income and food insecure older adults who attend the Fair <br /> Oaks Adult Activity Center. <br /> o Weekly Senior Brown Bag Program <br /> o Case management services for older adults who are homeless or at risk of homelessness <br /> in collaboration with the FOCC Human Services Specialists. <br /> 3.
