6.1.E. - Page 41 <br /> O�IG�NAL <br /> FUNDING A G R E E M E N T FQR HUMAN SERVICES FINANCIAL ASSISTANCE PROGRAM <br /> B�7W�L�N 7HE CfTY OF REDWOOD CITY AND <br /> FRESH LIFELINL�S FOR YOUTH <br /> THIS AGR�EMENT, made and entered into this day of , 2012, by and <br /> between the CiTY OF REDWOOD GI7Y, a charter city and a municipal corporatian of the State af <br /> California ("City") and Fresh Lifelines for Youth, a CaGfarNa nonprofit organization (°Contractor"); <br /> WITNESSETH: <br /> WHEREAS, Contractor will provide certain services through pragrams, including the program <br /> described in �xhibit"A" (°Contract�bjectives"), and <br /> WHEREAS, Contractar has requested financial assistance in order to conduct the �rogram for <br /> residents af City during the fiscal year commencing Jufy 1, 2012, and ending June 30, 2Q13; and <br /> WHEREP,S, City has reviewed said request and desires to allocate to Contractor #he sum of <br /> $�2,500; <br /> NQW, TNEREFORE, the parties hereto do hereby agree as fallow5: <br /> 1. FINANCIAL A551STANCE. City shall allocate to Contractor the sum af Twel�e Thousand <br /> Five Mundred Dollars for exclusive use by Contractor during the fiscal year commencing July 1, 2012, and <br /> ending June 30, 20�3, to be used by Contractar solely for the purposes described in paragraph 2. <br /> 2. USE OF FllNDS. Contrac#or shafl use the funds provided pursuant to paragraph 1 solely <br /> for the Program purposes described in Exhibit "A" (Contract Objecti�es) attached hereto and by this <br /> reference incorporated herein. <br /> 3. CHANG�S TO PROGRAM. No changes in the Program described in thi5 agreement which <br /> is funded by the financial assistance provided under paragrapF� 1 shall be mad� without the priar written <br /> consent of City. <br /> 4. PAYMENTS. Contractor shall keep detailsd and accurate records af all expenditures made <br /> and expenses incurred which are funded under this agreement.
