EXHIBIT"A" <br /> SCOPE OF SERVICES AND TIME SCHEDULE <br /> PROJECT DESCRIPTION: <br /> Fair Oaks Elementary School Synthetic Turf Replacement <br /> SCOPE OF SERVICES: <br /> A. Project Start-up: Cost: Not to exceed $4,500 <br /> 1. Consultant shall attend one kick-off meeting with the City to discuss project, <br /> including budget, timeline and parameters. Meeting agenda and reportwill be <br /> provided. <br /> 2. Available"as-builY'drawings or original specifications shall be provided by the City <br /> to Consultant. <br /> 3. Consultant shall use existing topograph ic survey and "as-built"drawings to prepare <br /> new construction drawings. <br /> 4. Consultant shall perform a final site walk for review to identify any residual site <br /> issues in need of renovation including but not limited to perimeter edging, nailing <br /> board, and base rock. <br /> 5. Consultant shal I remove a 24-inch square sample of the existing carpet out of the <br /> field of play and have tested by a lab for any material toxicity including but not <br /> limited to lead. <br /> A. Test results are to be provided to City for inclusion in Bid Package"B". <br /> B. City to repair the sam ple area. <br /> C. City to pay for the lab cost directly. <br /> 6. Consultant shall provide preliminary schedule for design, turf review, site <br /> construction and turf installation. <br /> 7. Consultant shall provide a letter report with final scope of recommended field <br /> renovation work. <br /> 8. Consultant shall attend one meeting with the City to present final recommendations <br /> for project implementation. Meeting agenda and minutes will be provided. <br /> B. Construction Documentation Cost: Not to exceed $12,250 <br /> (Including reimbursable) <br /> 1. Package"A"for Turf Installer <br /> A. 75% Design Submittal <br /> a. Consultant shall submit 75% Site Plan to scale with striping for a full <br /> size soccer field only. No cross striping for junior league play. <br /> b. Consultant shall review turf specification to be installed and <br /> recommend changes if appropriate. <br /> c. Consultant shall provide three (3) copies of the site plan to the City for <br /> review. <br /> B. 100% Design Submittal <br /> a. Based on 75% submittal comments, Consultant shall submit revised <br /> 100% Site Plan with final striping for soccer only field. <br /> 2.
