Company may propose report formats that are responsive to the objectives and <br /> audiences for each report. The format of each report shall be approved by <br /> Agency. <br /> Monthly reports shall be submitted within fifteen (15) calendar days after the end <br /> of the report month. Quarterly reports shall be submitted within thirty (30) <br /> calendar days after the end of the quarter. Quarters end on March 31, June 30, <br /> September 30, and December 31. Annual reports shall be submitted before <br /> January 31st following the reporting year. <br /> <br />9 7.3.2 Monthly Reports <br />10 The information listed shall be the minimum reported: <br />11 a) Green Waste Collected by Company by kind of Waste Generator, in tons <br />12 by material. <br />13 b) Complaint summary, for month and cumulative for report year, as above. <br />14 c) Narrative summary of problems encountered, and actions taken, with <br />15 recommendations for Agency, as appropriate. <br />16 d) Number of accotmts by category shown for each month of reporting year <br />17 and previous years, as above. <br />18 e) Participation rates in same format as number of accounts. <br />19 f) County Backyard Composting Program referrals made. <br />20 g) Materials Sales. Sales statement showing: kinds of material, quantity <br />21 collected and purchased (in tons), gross revenue per ton, and net sales <br />22 revenue. <br /> <br />23 7.3.3 Quarterly Reports <br />24 The infox-a-,ation listed shah be the minimum reported: <br />25 a) Quarterly summaries of the monthly infot-££tation listed in Section 7.3.2 of <br />26 this Agreement. <br />27 b) For the first year of the term of the Agreement, provide a quarterly <br />28 summary assessment of the overall Green Waste program from <br />29 Company's perspective relative to financial and operational status of <br />30 program. Subsequent to the first year, the summary assessment will be <br />31 made on an annual basis. The operational status is to relate to how well <br />32 the program is operating for efficiency, economy, and effectiveness <br />33 relative to meeting aH the goals and objectives of this Agreement.
