QRIGIN�� <br /> AMENDMENT NO. 1 TO GENERAL SERVICES AGREEMENT <br /> Universai Maintenance, Inc. <br /> T is Amendment No. 1 is entered into and effective as of the � day of <br /> �_, 2013, amending the agreement dated 2/28/13 (the "Agreement") by <br /> and b een the City of Redwood City, a charter city and municipal corporation ("City"), <br /> and I Service Maintenance, Inc., DBA Universal Maintenance, ("Contractor") <br /> (collectively, the "Parties")for cleaning and maintenance services. <br /> RECITALS <br /> A. On February 28, 2013, the Parties executed a General Services Agreement; <br /> and <br /> B. The Parties have negotiated and agreed to a supplemental scope of work and <br /> fee schedule, which is attached to and incorporated by this reference as Exhibits "A" <br /> (Docktown Marina — Facilities Maintenance Service) and "B" (Docktown Marina - <br /> Sewage Pumping Service). <br /> NOW, THEREFORE, in consideration of these recitals and the mutual covenants <br /> contained herein, the Parties agree as follows: <br /> 1. In addition to those services contained in the Agreement, as may have <br /> been amended from time to time, Contractor will provide those services described in <br /> Exhibits "A" and "B". <br /> 2. City will pay Contractor for all work associated with those services <br /> described in Exhibit "A" for the monthly sum of three thousand five hundred eighty eight <br /> dollars and seventy five cents ($3,588.75), and for all work associated with those <br /> services described in Exhibit "B" at the hourly rate of fifty five dollars per hour ($55.00 <br /> per hour - estimated at 4 hours per week) for an aggregate amount of ($58,937.50). <br /> Contractor will provide City, on a monthly basis, copies of invoices sufficiently detailed to <br /> include hours performed, hourly rates, and related activities and costs for approval by <br /> City. <br /> 3. Contractor will complete all work described in Exhibits "A" and "B" by <br /> August 10, 2013. <br /> 4. All other provisions of the Agreement, as may have been amended from <br /> time to time, will remain in full force and effect. <br /> 5. All requisite insurance policies to be maintained by Contractor pursuant to <br /> the Agreement, as may have been amended from time to time, will include coverage for <br /> this Amendment. <br /> 6.
