Exhibit A <br /> FACILITIES MAINTENANCE SERVICE <br /> Docktown Marina-Redwood City, CA <br /> This is a two (2) month agreement to begin on June 10, 2013 and end on August 10, 2013. This <br /> proposal will cover the Facilities Maintenance Service for the above referenced property. <br /> 1. SCOPE OF WORK: A uniformed worker &om Universal Maintenance will be on hand <br /> at the Docktown Marina five (5) days per week (Mon-Fri) for three (3) hours per day to <br /> perform safety inspections, minor repairs, and respond to a variety of other cleaning and <br /> maintenance needs as they arise. The areas to be maintained include the publicly <br /> accessible areas of the docks and marina, the public and/or City-owned buildings on the <br /> property, the public restrooms, mailboxes, laundry facilities, and parking areas. The <br /> maintenance worker's responsibilities will include,but are not limited to,the following: <br /> • Serve as the primary contact person interfacing with the On-Site Manager,Alex Kozlov,to <br /> coordinate and perform all routine cleaning work and minor repairs, including carpentry <br /> and minor repairs to lighting,electrical,and plumbing systems. <br /> • Communicate and coordinate with the designated City Public Works Managers for <br /> notification and discussion of major repair projects. <br /> • Pick up repair tags from Alex Kozlov and distribute to appropriate Public Works Manager <br /> for correcting the issues. <br /> • Provide on-call service for evening and weekend emergencies. <br /> • Assist with identifying persistent problems and interfacing with the On-Site Manager, <br /> Public Works Managers, and Universal Maintenance Account Manager to propose <br /> solutions. <br /> • Assist with the proper disposal of trash and recycling from the property and haul any large <br /> debris that was illegally left on the premises to the dump on a time-and-materials basis. <br /> • Handle routine cleaning of public restrooms and laundry facilities on the premises. <br /> 2. FEE: All the aforementioned work will be billed to City for the monthly sum of <br /> $3,588.75. The hourly rate for additional work done during normal business hours will <br /> be $55.00 per hour.
