Exhibit E PerFormance Measurement Performance Measures for this contract are under development based on the Core Agency Performance Measurement report. The County will distribute a copy of this report to all Core Agencies when measures are agreed upon by both parties. A notice confirming the agreed upon measures will be issued in the form of a letter to the contractor and included in the next FY contract. If the contract is amended prior to its conclusion, PerFormance Measures may be added at that time. Otherwise, Performance Measures will be added at the time of renewal, whichever comes first. Contractor Responsibilities Contractor will engage in activities and supply information required to accurately report on performance measures. Activities include, but are not limited to: • Attending planning and informational meetings; • Developing performance measures; • Collecting and entering data into the Clarity System and submitting any additional data necessary to fulfill measurement requirements; • Participating in technical assistance and training events offered by the Human Services Agency and seeking technical assistance and training necessary to fulfill measurement requirements; • Participating in a review of and interpretation of performance and outcome information; • Contractor will participate in HOPE (Housing Our People Effectively) activities as mutually agreed upon. HOPE is a ten-year action plan that brings together the business, non-profit and public sector communities to address the challenging issue of homelessness at its core, rather than manage it at the margins. Countv Responsibilities County,through the Human Services Agency, will: • Provide technical assistance and support to assist Contractor's Core Clarity Performance Measurement Report. • Conduct review of performance measures.
