EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> 2012 LINDEN PARK PROJECT <br /> PROJECT DESCRIPTION: <br /> Linden Park is a small neighborhood park located in the Redwood Oaks Neighborhood on the San <br /> Francisco Public Utilities easement.The play equipment and improvements in the park were removed to <br /> install the water pipeline improvements in 2010. The SFPUC leveled the lot and will issue a new <br /> encroachment permit for a new park. The large underground trench along the easement restricts the <br /> types of improvements that can be installed in the park. To build the new park, the City will undergo a <br /> community outreach process to generate a design, produce construction drawings, and obtain a <br /> revocable, long-term encroachment permit from the SFPUC for the recreational use of the easement. <br /> DESI6N SERVICES: <br /> The consultant will conduct preliminary site investigation; prepare and develop preliminary concepts <br /> plans; co-facilitate a community meeting where they will present the final concept plan to the <br /> community; prepare construction plans, specifications, and cost estimates suitable for bidding for the <br /> park project and assist the City during the construction phase of the project. <br /> TASK A—PRELIMINARY INVESTIGATION <br /> Consultant shall: <br /> 1. Collect and review all available background documents provided by City including: <br /> a. City SWPPP documents <br /> b. SFPUC documents <br /> c. Drawing and numbering standards for drawing sheets <br /> d. Title Biock in AutoCAD <br /> 2. Perform analysis and investigation of existing conditions including the location of SFPUC utilities, as <br /> well as other existing utilities, and site opportunities and constraints <br /> 3. Develop initial estimate of construction cost based on the plan presented during the interview <br /> 4. Attend one project kick-off ineeting w/City and SFPUC <br /> 5. Prepare AutoCAD base sheets for electronic survey file Coordinate soil fertility test to provide <br /> baseline soil amendment recommendations <br /> 6. Prepare a project schedule that includes approximate days for review by City and SFPUC <br /> Deliverable: <br /> a. Submit a project schedule one week from notice to proceed. <br /> b. Submit a summary of site analyses including the restrictions of improvement types and footing <br /> depths based on SFPUC requirements two weeks from notice to proceed. <br /> TASK B—COMMUNITY MEETINGS AND CONCEPTUAL DESIGN <br /> Consultant shall: <br /> 1.
