activity through a planned and developmentally appropriate physical education and nutrition education 6.1.C. - Pag2 7 <br /> instruction that meets the PE standards from kindergarten through 5th grade. <br /> Goal 5: Program Evaluation: SHD Program PE+ Program Manager will consult with staff from Applied Systems Research <br /> (ASR)to develop assessment tools and will work with CITY, PCC,and RCSD to conduct baseline assessments and <br /> ongoing monitoring and reassessments. It is anticipated that RCSD Wellness Coordinator will assist with survey <br /> execution and data collection. Data will be used to measure program quality and impact,share outcome information <br /> with stakeholders,make necessary modifications,and use best practices to cultivate and expand program. <br /> Goal 6:Sustainability: Program is designed to assure positive changes are sustained. <br /> Goal 7: Program Growth: In coordination with PCC,the program is expected to expand program to all 14 elementary schools <br /> by year 3 with financial contributions from additional sources. <br /> V.Staffing Plan and Employee Contracts <br /> CITY will maintain a PE Coordinator and approximately 12 coaching staff with the funds provided by SHD through this agreement. <br /> CITY will ensure that all coaches are LiveScan fingerprinted, and TB-tested, subject to the final approval of RCSD to work on <br /> school grounds. Coaches will be at least the age of majority, and have graduated from high school. Coaches will have either <br /> worked with children before or coached sports in other contexts and will all be trained to teach physical education using PE+ <br /> curriculum. <br /> CITY will be responsible for advertising, interviewing, and filling these positions. Contractual agreements, including consultant or <br /> employment agreements for staff,will be made through CITY.The amount of SHD funding allocated for PE Coordinator includes <br /> salary and benefits. If City offers salary and benefits above the funding amount established by SHD pursuant to this Agreement, <br /> SHD will not be responsible for funding these additional amounts. SHD will not be responsible for any salary, benefits, insurance <br /> or other employee costs and shall not be deemed to be an employer of the personnel hired by CITY. <br /> All staff supported through the Healthy Schools grant will share in the responsibilities for attaining the goals and objectives outlined <br /> in the program proposal. Input from relevant staff will be sought in the implementation and evaluation of this program. <br /> VI. Distribution of Functions <br /> CITY's PE Coordinator and coaches will work with appropriate school staff to implement and support specific modules of the PE+ <br /> Program.The following staff positions and distribution of roles and responsibilities are outlined below: <br /> CITY PE+ Coordinator:The PE Coordinator supported through this grant will organize, implement,and provide on-site <br /> supervision of the PE+ program.This person is responsible for hiring,training,scheduling,supervising,and evaluating <br /> PE coaches and necessary personnel to ensure successful implementation of a safe and effective program. The PE <br /> Coordinator is also responsible for meeting with school administrators, teachers, partner organizations, and others as <br /> necessary to properly represent the CITY and the Program.The PE+ Coordinator will also be held responsible for the <br /> development, implementation,evaluation,and reporting of Program curriculum and results. <br /> CITY PE+ Coaches:The parties understand that CITY's physical education coaches will work on site during regular <br /> school hours and under the direction of RCSD credentialed teachers.Physical education coaches help children develop <br /> physical abilities and healthy habits that can last for the rest of their lives.The PE coaches supported through this grant <br /> will organize games and challenges that promote physical activity among children in grades K-5.Their goal is to develop <br /> motor skills and physical development among younger children and proper exercise and eating habits among older <br /> children. PE teachers must also adjust their curricula to adapt to children with disabilities or different physical abilities. <br /> The PE coaches will be knowledgeable about all activities provided to the children, be able to communicate and <br /> demonstrate skills properly, observe all their students and maintain discipline within the class. In addition to students, <br /> they interact with parents,other teachers and school administrators.There will be four coaches per at each school site <br /> when classes are taught; 1 head coach and 1 assistant coach.Coaches work no more than 25 hours per week(approx. <br /> 5 hours/day. Head coach works no more than 27 hours per week(approx.5.5 hours/day) <br /> VII.
