EXHIBIT D 09/09/2013 <br /> MONITORING VERIFICATION <br /> IDENTIFIED IMPACT RELATED MITIGATION MEASURE Implementation Monitoring and Timing Signature Date <br /> Pe1'fOrmanCe Crlterla Entity Verification Entity Requirements <br /> project applicant shall provide an <br /> Archaeological Monitoring and Data <br /> Recovery Program(AMDRP)prepared by a <br /> qualified archaeologist and outlining <br /> procedures for resources encountered <br /> during construction. The AMDRP shall <br /> include specific measures to insure <br /> compliance with State Public Resources <br /> Code section 5097.98 and CEQA <br /> Guidelines section 15064.5(d)and(e)in <br /> the event that human remains are <br /> encountered. <br /> (3) A qualified archaeological monitor will <br /> visit the site for spot-checks during <br /> excavations exceeding five feet in depth <br /> below current grade. <br /> If subsurface archaeological resources are <br /> encountered,excavation shall halt in the <br /> vicinity of the resources,and the <br /> archaeological monitoring shall evaluate <br /> the resource and its stratigraphic context. <br /> The monitor shall be empowered to <br /> temporarily halt or redirect construction <br /> activities to ensure avoidance of adverse <br /> impacts to archaeological resources. <br /> If disturbance of an archaeological <br /> resource cannot be avoided,the mitigation <br /> program described in the AMDRP, <br /> including measures set forth in the City's <br /> CRMP and in compliance with sections <br /> 15064.5 and 15126.4 of the CEQA <br /> Guidelines,shall be implemented. <br /> If subsurface paleontological resources are <br /> encountered,excavation shall halt in the <br /> vicinity of the resources and the project <br /> paleontologist shall evaluate the resource <br /> and its stratigraphic context. The monitor <br /> shall be empowered to temporarily halt or <br /> redirect construction activities to ensure <br /> avoidance of adverse impacts to <br /> paleontological resources.
