Sequoia Healthcare District <br /> and <br /> Redwood City Parks, Recreation, and Community Services ("City") <br /> Sequoia Healthcare District is pleased to offer Redwood City Parks, Recreation, and <br /> Community Services a one-time budget increase of$25,322 from the current PE+ grant <br /> amount of$278,925 to be put toward: <br /> 1. $20,000 toward implementation of PE+ recess and lunch staff support at the four <br /> PE+ schools: Garfield, Hawes, Henry Ford, and Selby Lane; five days per week <br /> for the full school year. <br /> 2. $3591 to cover additional funding need, a budget difference due to changes in <br /> schedules for classes and increased recess/lunch support. <br /> 3. $1731 evaluation and data entry support by a qualified PE+ coach. (100 hours x <br /> $17.31/hour head coach pay rate). <br /> Revised budget is attached. <br /> A revision to Goa11 in the MOU to match changes in program and budget is as follows: <br /> Goa11: Target Schools -City will conduct programming at the four schools listed below <br /> during the 2013-14 academic year. If opportunity presents itself to expand to other <br /> schools during this grants cycle, an amendment to this agreement will be made. The <br /> budget is based on the following parameters: <br /> • 180 school days in the year. Length of school day is dependent on each school's <br /> individual class schedule (approximately 6.5 hours/day). <br /> • 152 days of PE+ "full program" implementation that includes PE curriculum <br /> instruction two days per week for each class grades K-5 by four total coaches <br /> (two coaches per class) and recess/lunch support five days per week with a <br /> minimum of 2 coaches. The "full program" curriculum instruction ends May 23, <br /> 2014. <br /> • 23 days of"recess/lunch support only", without curriculum instruction, by two <br /> coaches during the first two weeks of school, two weeks of testing (previously <br /> Star Testing), and one week following the end of the "full program" curriculum <br /> instruction. "Recess/lunch support only" ends May 30, 2014. <br /> To acknowledge this agreement and accept the budget increase, please sign below and <br /> fax to Pamela Kurtzman at 650-421-2159. <br />
