4.1.C. - Page 18 <br /> EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Middlefield Road Underground Utility District 2011-1 Project <br /> Design Scope of Work <br /> PROJECT QESCRIPTIO�V: <br /> Consultant shall provide design and engineering services for the Middlefield Road Underground <br /> Utilities District 20�1-1 Project. �tte o6jectives af the Middlefield Road Ur�de€'ground Utili�ies District <br /> 2011-1 Project are ta rem�ve existing averhead utility structures; install facilities for supplying electric, <br /> co�nmunicatian, and similar services �nderground; and make stree�seape irrtprovements that increase <br /> pedestrian and bicyclist's mobility. The project area is defined as ihe length of Middlefield Road <br /> between Main Street and MacArthur A�enue. <br /> Far t�e first phase of t�is agreement, the Consultar�t shall specifitally pravide design and engineering <br /> services for this projec�to include aerial topography, utility surveying, legal description, and produetion <br /> of the Jaint Trench Intent Drawing. 7he Joint Trench Entent prawing shall show the City's preferred <br /> proposed utifity trench alignment and the street layout. The atfected utility cor�panies' (PG&E, AT&7, <br /> Comcast, Astound) will use these loint Trench lr�tent drawings to prepare their indi�idual utility desi�n <br /> far their underground facilities. Based on distussions with PG&E, their design will take up to two years <br /> ta cor�tplete(5ummer 2014). <br /> tJpan corr�pletion of the utility comparties design for their facilities, to the City may arr�end this <br /> � agreernent to include finalizing the tamposite joinfi trench drawings, preparing "Form B", finalizing the <br /> � streetscape design, and constructiort st�pport services. <br /> DESIGN: <br /> TA5K 1—AERIAL TOPOGRAPHY <br /> Consu{tant shall: <br /> 1.
