Exhibit A—Scope of Services & Contractor Quote <br /> 1) Background: <br /> The City of Redwood City, California ("City") has solicited and received the attached bid (pages <br /> 10-16 of this Agreement) from qualified commercial carpet installer H.V. Walker Co. Inc. <br /> ("Contractor") to replace carpet and cove base for the Childcare Center and Senior Center <br /> located within the Fair Oaks Community Center (2600 Middlefield Road, Redwood City, CA <br /> 94063), with Contractor's bid hereby incorporated into this Scope of Services. Contractor will <br /> furnish, install and provide all Interface manufactured carpet squares and ancillary materials <br /> needed to provide a complete product. The Childcare Center and Senior Center (located within <br /> the Fair Oaks Community Center) have slightly different requirements,therefore,the Contractor <br /> will work closely with Redwood City staff during the entire project.All areas within the Childcare <br /> Center and Senior Center shall be returned to their original condition, including furniture, <br /> computers, desks, chairs, equipment, supplies, etc. prior to the end of the Contractors work <br /> shift. <br /> 2) Site Description <br /> Carpet Replacement will take place within the Fair Oaks Community Center in the: <br /> i) Childcare Center <br /> (1) Infant Care Center(Rest Area & Office) <br /> (2) Childcare Modular Building (Office & Children's Area) <br /> ii) Senior Center <br /> 3) Preferred Installation Dates and Hours <br /> The normal hours of operation for the Childcare Center and Senior Center are generally Monday- <br /> Friday, 8am to 4pm. Work will not occur during these normal hours of operation so that the <br /> participants of the Childcare Center and Senior Center are not displaced. <br /> Listed below are the preferred installation dates, in order of preference. <br /> 15t Choice <br /> Senior Center: Friday, May 23 (starting at 1:30pm)till Sunday, May 26 (till work is complete). <br /> Childcare Center: Friday, May 30(starting at 1:30pm)till Sunday,June 1 (till work is complete). <br /> 2nd ChoiCe <br /> Senior Center and Childcare Center: Friday, May 30 (starting at 1:30pm) till Sunday, June 1 (till <br /> work is complete). <br /> 3�d ChoiCe <br /> Senior Center and Childcare Center: Friday, June 6 (starting at 1:30pm) till Sunday, June 8 (till <br /> work is complete). <br /> PUBLIC WORKS DIVISION 9 <br /> Carpet Replacement—Fair Oaks Community Center(FOCC) <br /> ATTY/AGR/2014.068/H.V.WELKER CO.
