��.� <br /> � � 1� <br /> w��t�=� <br /> South Bayside Waste <br /> Management Authority <br /> A Public Agency <br /> SBWMA APPLICATION FOR REDWOOD CITY TO PARTICIPATE IN DOOR-TO-DOOR <br /> HOUSEHOLD HAZARDOUS WASTE, UNIVERSAL WASTE, ELECTRONIC SCRAP AND <br /> SHARPS COLLECTION, RECYCLING AND DISPOSAL SERVICES PROGRAM <br /> JANUARY 2015 <br /> INFORMATION ABOUT THE PROGRAM <br /> Description: SBWMA has contracted with WM Curbside LLC, At Your poor Special Collections, <br /> (formerly Curbside, Inc. (Contractor)), a firm specializing in the door-to-door collection, recycling <br /> and disposal of household hazardous waste, universal waste, e-scrap and sharps. SBWMA <br /> engaged Contractor pursuant to a competitive Request for Proposals (RFP) process. <br /> Qualifications: Contractor is one of the few companies that currently provide the comprehensive <br /> scope of services specified in the SBWMA's RFP. On October 22, 2009, the SBWMA Board of <br /> Directors authorized SBWMA to execute a master contract with Contractor and serve as the <br /> contract administrator to facilitate Member Agency's participation in the program. <br /> SBWMA Facilitation of the Program: SBWMA will distribute information about the Program and <br /> applications to participate in the program to its Member Agencies. Interested SBWMA Member <br /> Agencies will return the application to SBWMA, indicating their desire to participate and the cost <br /> they are expected to pay for the program. SBWMA will forward the names and contact persons <br /> at the Member Agencies that wish to participate to Contractor so it can establish the details of <br /> the Program with the Member Agency. <br /> At SBWMA's direction, Contractor will inform each Member Agency which subscribes to the <br /> program of the date of commencement of service. Contractor will provide services to each <br /> participating member. Contractor will submit monthly reports and invoices to SBWMA. SBWMA <br /> will compensate Contractor for providing the program. Agency will reimburse SBWMA for its <br /> participation in the program. If required, Agency will pramptly direct the Collection Contractor <br /> (i.e., Recology San Mateo County) to remit payment to SBWMA to ensure SBWMA invoices are <br /> paid within thirty (30) days. <br /> The Agency must provide SBWMA sixty (60) days notice of its decision to provide additional <br /> service to multi-family residences.
