The goals (Exhibit A) and budget (Exhibit <br /> B) are attached. In order to assure compliance with applicable provisions of the Internal <br /> Revenue Code, we must obtain the Grantee' s written agreement to each of the following terms <br /> and conditions of this grant: <br /> ( 1 ) All of the grant funds are to be used for, and only for, the purpose of supporting the <br /> SPIN/Shinnyo Fellow. Use of any portion of the grant funds, including any interest <br /> earned thereon, for any other purpose must have the prior written approval of the <br /> Foundation. <br /> (2) The grant funds must be used only in accordance with the budget submitted as part of <br /> your grant request, a copy of which is attached to this letter as Exhibit B. Any <br /> changes in the budget or any modifications to other grant conditions must be <br /> authorized by prior written approval of the Foundation. <br /> (3) Narrative report(s) and corresponding financial statement(s) must be furnished on the <br /> date(s) indicated herein on the progress made in accomplishing the purpose of the <br /> grant. Financial statements must describe the expenditures of the grant funds in <br /> accordance with the submitted budget.
