iiW <br />Sequoia <br />Healthcare <br />District <br />525 Veterans Blvd. <br />Redwood City, CA 94063 <br />ORIGINAL <br />New Grants Ft Programs Agreement 2016-2017 <br />On October 5, 2016, the Sequoia Healthcare District Board of Directors approved a grant to the City <br />of Redwood City Parks, Recreation and Community Services. The grant is for the explicit purposes <br />stated below and is subject to the following conditions. To acknowledge this agreement and accept <br />the grant, please have an officer of your organization read and sign this agreement and return the <br />original to us at Sequoia Healthcare District, 525 Veterans Blvd., Redwood City, CA 94063. <br />Grant Number: <br />Grant Amount: <br />Grant Period: <br />Grant Purpose <br />Payment Schedule: <br />Reporting Schedule: <br />Special conditions: <br />I. Expenditure of Funds <br />2016-17 New Grants 01 <br />$400,000 <br />October 5, 2016 -June 30, 2017 <br />Magical Bridge at Red Morton Park <br />First payment January 2017 <br />Second payment June 2017 <br />1) First report due May 2017 - Interim Grant Report <br />2) Final report due December 2017 - Final Grant Report <br />Sequoia Healthcare District Representative may request a site visit <br />during the grant cycle. <br />The grantee shall use this grant solely for the purposes stated above, and the program may be <br />modified only with the written approval of the Sequoia Healthcare District's Board of Directors. <br />The grantee shall return to the District any unexpended funds: <br />1) at the end of the grant period, or <br />2) if the Sequoia Healthcare District determines in its reasonable discretion that the grantee <br />has not performed in accordance with this agreement and the approved Magical Bridge at <br />Red Morton Park project, or <br />The grantee shall maintain adequate supporting records consistent with generally accepted <br />accounting practices to show the use of these funds. The grantee shall make these books and records <br />available to the Sequoia Healthcare District in keeping with the grant reporting schedule described <br />below. <br />The grantee shall not use any portion of the funds granted herein to influence the outcome of any <br />specific public election.
