<br />DUPliCATE <br />ORIGINAL <br /> <br />Memorandum of Understanding <br />California Association of Food Banks <br />Food Stamp Outreach Program 2006-07 <br />Contract No: 06-55070 <br /> <br />This is a Subcontracting Agreement of the California Food Stamp Access Improvement Project of <br />the California Department of Health Services and the California Department of Social Services <br /> <br />Between <br /> <br />Second Harvest Food Bank of Santa Clara & San Mateo <br />(a member of the California Association of Food Banks) <br /> <br />and <br /> <br />Fair Oaks Community Center <br />(Name of Subcontractor Organization) <br /> <br />1. Definitions <br /> <br />1.00 Association: California Association of Food Banks, 909 12th Street, #203 Sacramento, <br />CA 95814 <br /> <br />1.01 Subcontractor: not-for-profit organization that has submitted a Food Stamp Outreach <br />Program Approved Scope of Work (SOW) and Budget. <br /> <br />II. The Contract Documents <br /> <br />(A) Approved Scope of Work <br /> <br />(B) Approved Budget Justification <br /> <br />(C) List of Allowable Activities <br /> <br />2.01 This Agreement, upon execution by the authorized signatories, supersedes all prior <br />negotiation, representations and agreements either written or oral. <br /> <br />2.02 Two (2) originals of this Agreement shall be signed by the SHFB of Santa Clara & <br />San Mateo and the Subcontractor. <br /> <br />III. Contract Time <br /> <br />3.00 The Contract Time is the period within which the project obligations of this <br />Agreement shall be performed. The Contract Time commences October 1, 2006 and <br />terminates September 30,2007. <br />
