The goals (Exhibit A) and budget (Exhibit are attached. In order to assure compliance <br />with applicable provisions of the Internal Revenue Code, we must obtain the Grantee's written <br />agreement to each of the following terms and conditions of this grant: <br />(1) All of the grant funds are to be used for, and only for, the purpose of supporting the <br />SPIN/Shinnyo Fellow. Use of any portion of the grant funds, including any interest <br />earned thereon, for any other purpose must have the prior written approval of the <br />Foundation. <br />(2) The grant funds must be used only in accordance with the budget submitted as part of <br />your grant request, a copy of which is attached to this letter as Exhibit B. Any changes <br />in the budget or any modifications to other grant conditions must be authorized by prior <br />written approval of the Foundation. <br />(3) Narrative report(s) and corresponding financial statement(s) must be furnished on the <br />date(s) indicated herein on the progress made in accomplishing the purpose of the grant. <br />Financial statements must describe the expenditures of the grant funds in accordance <br />with the submitted budget.
