REV: 06-22-22 RL City of Redwood City/Fair Oaks Community Center and Help a Mother Out FYE 23 Service Agreement This Service Agreement (the “Agreement”) is made and entered into as of the date set forth on the signature page below, by and between Help a Mother Out (“HAMO”), a California nonprofit public benefit corporation, and City of Redwood City (“Community Partner”). WHEREAS, HAMO desires to donate diapers and other goods to Community Partner, and Community Partner desires to accept the donation; NOW, THEREFORE, in consideration of the mutual agreements and conditions contained herein, and other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties hereby agree as follows: 1)Community Partnership. a) HAMO shall provide the following: i) Priority distribution of free diapers or other goods (including, but not limited to, pull ups, baby wipes, potty training supplies, or other essential family care items) to Community Partner. Diaper brands distributed to Community Partner may vary and HAMO cannot accommodate requests for specific brands of diapers. ii) Advanced scheduling of distributions, which shall take place on a monthly or quarterly basis, based on (i) available inventory and (ii) Community Partner’s election to be either a “Gold” or “Silver” Partner (as such terms are defined in paragraph 2 below). iii) Community Partner’s name listed on HAMO’s website www.helpamotherout.org. iv) Community Partner’s name listed on the printed materials for HAMO’s annual fundraising event(s). b) Community Partner shall provide the following: i) A nominal annual service fee paid to HAMO in the amount of One Thousand Eight Hundred Seventy Five Dollars ($1,875.00) for service during the term of July 1, 2022 – June 30, 2023. ii) Free distribution of diapers and other goods only to families experiencing distress, exclusively through the following program(s): (1) Information and Referral Program iii) Community Partner hereby certifies the above programs collectively serve an estimated 1,800 children per year. 2)Diaper Program Enrollment: Gold Partners and Silver Partners.
