REV: 07-28-25 LF <br />Caring Community Grants Grant Agreement <br />On June 4, 2025, the Sequoia Healthcare District's Board of Directors approved a Caring Community grant <br />to Redwood City Together, and City of Redwood City acting as fiscal agent for Redwood City Together <br />("Grantee"), for Promoting Community Health with Youth Promotores. To acknowledge this agreement and <br />accept the grant, please have an officer of your organization read and sign this agreement. <br />Grant Funding:$45,000.00 per year for two years <br />Grant Period:July 1, 2025 - June 30, 2027 <br />Payment Schedule:Year 1 First payment: July, 2025 <br />Year 1 Second payment: February, 2026 <br />Year 2 First payment: July, 2026 <br />Year 2 Second payment: February, 2027 <br />Reporting Schedule:1) Final Report Year 1: June 30, 2026 <br />2) Final Report Year 2: June 30, 2027 <br />I. Expenditure of Grant Funding <br />Grantee shall use grant funding exclusively for the purposes described in the final Letter of Intent (LOI) <br />and/or full application submitted by the Grantee through Sequoia Healthcare District’s (“SHD”) grant portal. <br />Any modifications to the approved LOI or full application must be requested in writing and are subject to <br />review and approval by SHD through the execution of a Scope of Work Change Form, as accepted by SHD. <br />The Grantee shall return to SHD any unexpended grant funds at the end of the grant period. <br />SHD reserves the right, in its sole discretion, to retract, withhold, or discontinue funding if it determines that <br />any act, omission, or event has occurred that compromises the intended use of the grant, violates the terms of <br />the grant agreement, applicable laws or regulations, or otherwise necessitates the return or suspension of <br />funds. <br />SHD shall have the right, upon reasonable notice, to inspect, audit, and copy any and all books, records, <br />accounts, and supporting documentation maintained by the Grantee that relate to the receipt, use, and <br />expenditure of SHD grant funds. The Grantee agrees to retain such records for a minimum of seven (7) years <br />following the conclusion of the grant term or as otherwise required by law. <br />Grantee shall not use any portion of the funds granted herein to influence the outcome of any specific public <br />election.
