- t��IGIN�L <br /> AGREEMENT <br /> CENTER FOR INDEPENDENCE OF INDIVIDUALS WITH DISABILITIES <br /> (Housing Accessibility Modifications (HAM)) <br /> THIS AGREEMENT, made and entered into this ( day of <br /> 014, by and between the CITY OF REDWOOD CITY, a charter <br /> city and municipal corporation of the State of Califomia ("City") and CENTER <br /> FOR INDEPENDENCE OF INDIVIDUALS WITH DISABILITIES, a California <br /> nonprofit organization ("Contractor"; collectively the "Parties"). <br /> WITNESSETH: <br /> WHEREAS, Contractor provides certain services through programs, <br /> including the program described in Exhibits "A" ("Program & Quantitative Goals"), <br /> and "B" ("Program Budget"), attached hereto and made a part hereof; and <br /> WHEREAS, Contractor has requested financial assistance in order to <br /> conduct the program described in Exhibit A for residents of City during the fiscal <br /> year commencing July 1, 2014, and ending June 30, 2015 (the "Fiscal Year"); <br /> and <br /> NOW, THEREFORE, the Parties hereby agree as follows: <br /> 1. FINANCIAL ASSISTANCE. City will allocate to Contractor the sum <br /> of Eighteen Thousand Five Hundred and No/100 Dollars ($18,500) for exclusive <br /> use by Contractor during the fiscal year commencing July 1, 2014, and ending <br /> June 30, 2015, solely for the purposes described in Paragraph 2. <br /> 2. USE OF FUNDS.
