17. The developer shall post Performance and Labor & Materials Bonds for all <br /> improvements to be dedicated to the City. <br /> 18 . The applicant/contractor shall apply for an Encroachment Permit from the <br /> Community Development Department (CDD) for work in the City public <br /> right-of-way, easements or property in which the City holds an interest. <br /> 19. A pavement overlay was performed on Kentfield Avenue in 2011 -2012 , <br /> and is subject to a pavement moratorium . Per Chapter 29 of the City's <br /> municipal code , no cuts in the street are allowed within 5 years of the <br /> completion of the pavement project. <br /> a . An exception may be granted by the City Engineer pursuant to the <br /> findings in Chapter 29 . If an exception is granted , the permittee <br /> would be required to reconstruct the street completely rebuilding , <br /> by removing all the pavement and aggregate base course material , <br /> re-compacting the sub-base and restoring the base material and <br /> then completely re-paving the street, for a minimum of the full width <br /> of all lanes impacted by the cut(s) (outside lane includes to the <br /> curb) and extending the entire length of the recent reconstruction <br /> by the City, which in this case is from Woodside Road to Redwood <br /> Avenue , or for a length determined by the City Engineer to be <br /> sufficient to restore the street to its new condition , as if it had never <br /> been cut, or as more specifically directed by the City <br /> Engineer/designee . <br /> b . For recently renovated streets the City Engineer may allow the <br /> permittee to restore the street to its former condition by resurfacing <br /> the street to the same extent as was performed by the City, except <br /> that the asphalt or surface material installed by the City must first <br /> be removed by the permittee to a depth as directed by the City <br /> Engineer. <br /> c. Alternatively, a permittee may choose to pay the Pavement <br /> Damage Restoration Fee established by City Council Resolution , in <br /> which event, the City Engineer shall authorize a repair less than full <br /> and complete reconstruction of the entire newly reconstructed <br /> street. <br /> 2O . Applicant shall apply for and obtain a Grading Permit. Prior to Grading <br /> Permit issuance , the applicant shall submit a Plot and Finished Grading <br /> Plan prepared by a California registered Civil Engineer. <br /> 21 . Project must meet fire flow requirements as established by the Fire <br /> Department which are based on the Fire Code.
