This task <br /> has an allowance for 3 legal descriptions. <br /> TASK 19—BOILERPLATE AND SPECIAL PROVISIONS <br /> In addition to the technical specifications, Consultant will assist City with the boilerplate specifications <br /> and the special provisions. This task will include making the first pass at editing the City standard <br /> boilerplate specifications and the special provisions with project-specific information. Many of the <br /> items will require discussion and coordination with City staff for administrative requirements included in <br /> the contract documents. City will prepare the final version of the boilerplate specifications and the <br /> special provisions prior to advertising the project. <br /> TASK.20—PROGRAM SCHEDULING <br /> Design Team will maintain a master schedule of the various projects and phases related to the MUUD <br /> project: City MUUD design, PG&E service trenches, City MUUD contractor, City streetscape design and <br /> construction,City street lights,utility companies pull wires&energize,and power pole removal. <br /> OPTIONAL SERVICES(OS) <br /> Listed below are services not included in the basic contract work but may be added upon written <br /> authorization from the City Engineer: <br /> OS/—POTHOLING <br /> Consultant will retain a potholing contractor to expose existing utilities at select locations. Potholing <br /> contractor will follow City requirements for working hours and submit traffic control plans to the City for <br /> approval prior to any lane closures on public streets. The potholing contractor will mark a set of plans to <br /> indicate depth, diameter and location of existing pipe. Budget included in this proposal assumes 15 <br /> potholes. <br /> 0S2—SWPPP <br /> Prepare a Storm Water Pollution Prevention Plan (SWPPP) by a Qualified SWPPP Developer (QSD) in <br /> conformance with California State Water Resource Control Board's Construction General Permit (CGP). <br /> SWPPP will be prepared per the CASQA Template and will be uploaded to the State's SMARTS System <br /> website. The City will provide information regarding Legally Responsible Person (LRP) and Approved <br /> Signatory (AS) for the project. Consultant will calculate the Risk Level based on the project type. This <br /> task does not include QSD and Qualified SWPPP Practitioner (QSP) services after uploading of the <br /> SWPPP to the SMARTS System. QSD and QSP services during construction will be part of a separate <br /> agreement. The City will be responsible for paying the fees associated with the CGP and submitting to <br /> the State Water Board. Construction may not begin until a WDID number has been issued by the State <br /> Water Board. <br /> 0S3—SIGNAL MODIFICATIONS <br /> MUUD project may require modifications to signals at various locations due to disruption of detector <br /> loops, and/or requirements for pedestrian push buttons due to the relocation of curb ramps at <br /> temporary bulbouts. The following intersections to be checked for signal conflicts include: Maple, <br /> Chestnut,Woodside,Willow,Charter and Douglas. <br /> 054—CALTRANS SUBMITTALS <br /> Shortly after the 65% submittal, Consultant will prepare a "100% Permit Submittal" for Caltrans review. <br /> The 100% permit submittal will be limited to the segment of joint trench across Woodside Road which is <br /> REV:02-24-15 VR <br /> Page 17 of 20 <br /> ATTY/AGR.2015.037/Bellecci&Associates-Middlefield UUD <br />
