The <br /> Ca!trans review package will be submitted separately and will be a separate plan series in the overall <br /> Joint Trench plan set. We assume three (3) Caltrans submittals prior to approval. District 4 Ca!trans will <br /> likely require the following forms to be completed as part of the Caltrans Encroachment Permit: Storm <br /> Water Data Report (SWDR), Preliminary Engineering Evaluation Report (PEER), Right-of-Way <br /> Certification, and Utility Certification. For all of these documents we assume they are the short-form <br /> versions with many boxes checked as"not applicable." <br /> OS5—SAN MATEO COUNTY JOINT TRENCH COMPOSITE DRAWINGS <br /> The City and County may enter into a separate agreement to add County composite drawing design <br /> services into this Agreement between Consultant and the City. Authorization of Task 0S5 to perform <br /> County design services will not commence until the separate agreement between County and City is <br /> approved by the City Engineer. The City/County agreement prepared by others will a) define the cost <br /> sharing for design services, b) method of payment from County to City, and c) may also address the cost <br /> sharing between County and City for construction. If Task 0S5 is activated,the scope of work will be to <br /> prepare the Composite Drawings for the San Mateo County joint trench located within San Mateo <br /> County Phase 3 MUUD Boundary, as shown on the Joint Trench Intent Drawing, dated September 2013, <br /> The level of detail and plans will be the same as described in basic services above. The fee for Task 0S5 <br /> assumes the NTP for this work occurs prior to Task 7A above.
