�,�.�.,,�., _ � __ _ ___ __ _ <br /> 6 1.Bya Page 8am Eva�ration:Ail <br /> �. �y partners from PCC,PRCS,RCSD,and SHD wiil waic cogether ro <br /> devebp and im�ement an evaiva�on plan that measur+es quality and impact.Part�ers w�l determine the <br /> des�ed outcomes of the program,adequately and continuousiy evaluate the�xogram using quantitative <br /> and qualitative assessment tools such as fitness assessments and surveys, impiement�propriate data <br /> callection and entry,use the ir�ormation to improve and modify the�ogram,ensu�that positive changes <br /> and outoomes a�sustained,and share the infortna6on with stakeholders in the folbwing areas: <br /> • student physicai fiNess,social-emotional factors,nutrition measures,and satisfacctian with PE+ <br /> • teacher/schooi official observations of student outcomes and satisfaaion with PE+ <br /> • coach competency and outoomes <br /> V.Staffing Plan and Employee Contrects <br /> CiTY will maintain a PE Coordinator and appropriate number of coaching staif with the funds provided by <br /> SHD through this agreement CITY will ensure that all voaches are LiveScan fingerprin�d,and TB-tested, <br /> subjec;t to the fina!approval of RCSD to work on school grounds.Coaches will be at least the age of <br /> majority,and have graduated from high school.Coaches wili have either waked with ch�dren before or <br /> coached sports in other cantexts and wiil all be trained to teach physical education using PE+curriculum. <br /> CiTY will be re.spons�le for advertising,interviewing,and fiUing these positions.Contractual agreements, <br /> including oonsultant or employment agreements for staff,wi�be made through C(TY.The amount of SHD <br /> funding albcated i�r PE Coordinator incl�les satary and benefits. If City oifers s�ary and benefits above <br /> the funding amount established by SHD pursuant to this Agreement,SHD will not be responsible for <br /> funding these addi�onal amaunts.SHD wip not be responsible for any salary,benefits,insurance or other <br /> employee casts and shall nat be deemed to be an empbyer of the personnel hired by CITY. <br /> Ail staff supparted through the Heald�y Sd�ools grant will share in the responsibditi�s for attaining the goals <br /> and objectives oufilined in tl�e program proposal. Input from relevant staff will be sought in d�e <br /> impieme�rtation and evaluatan of this program. <br /> VI.
