IDENTIFICATION OF KEY PERSONNEL <br /> 3_2 Vendor will designate one senior staff individual who will represent the awarded Vendor to NJPA. <br /> This contact person will correspond with members for technical assistance, questions or problems that <br /> may arise including instructions regarding different contacts for different geographical areas as needed. <br /> 3.3 Individuals should also be identified (if applicable) as the primary contacts for the contents of this <br /> proposal,marketing, sales, and any other area deemed essential by the Proposer. <br /> C.PROPOSER'S EXCEPTIONS TO TERMS AND CONDITIONS <br /> 3.4 Any exceptions, deviations, or contingencies a Proposer may have to the terms and conditions <br /> contained herein must be documented on Form C. <br /> 3_5 Exceptions, Deviations or contingencies stipulated in Proposer's Response, while possibly necessary <br /> in the view of the Proposer,may result in disqualification of a Proposal Response. <br /> D. FORMAL INSTRUCTIONS TO PROPOSERS <br /> 3.6 It is the responsibility of all Proposers to examine the entire RFP package, to seek clarification of any <br /> item or requirement that may not be clear and to check all responses for accuracy before submitting a <br /> Proposal. Negligence in preparing a Proposal confers no right of withdrawal after the deadline for <br /> submission of proposals. <br /> 3.7 All proposals must be sent to "The National Joint Powers Alliance®, 200 1st ST NE Staples, MN <br /> 56479." <br /> 3.8 Format for bid response: All proposals must be physically delivered to NJPA® at the above address <br /> in the following format: <br /> 3.8.1 Hard copy Original signed, completed, and dated forms C,D, E,H, and I from this RFP, <br /> 3.8.2 Hard copies of all addenda issued for the RFP with original counter signed by the Proposer, <br /> 3.8.3 Certificate of insurance verifying the coverage identified in this RFP, <br /> 3.8.4 Two complete copies of your response on A CD (Compact Disc) or flash drive containing <br /> completed Forms A,B,C,D,E,H, and I, and your statement of products and pricing together with <br /> all appropriate attachments <br /> 3.9 All Proposal forms must be legible. All appropriate forms must be executed by an authorized <br /> signatory of the Proposer. Blue ink is preferred for signatures. <br /> 3.10 Proposal submissions should be submitted using the electronic forms provided. If a Proposer chooses <br /> to use alternative documents for their response, the proposer will be responsible for ensuring the content <br /> is effectively equal to the NJPA form and the document is in a format readable by NJPA. <br /> 3.11 It is the responsibility of the Proposer to be certain the proposal submittal is in the physical <br /> possession of NJPA on or prior to the deadline for submission of proposals. <br /> 3.11.1 Proposals must be submitted in a sealed envelope or box properly addressed to NJPA and <br /> prominently identifying the proposal number, proposal category name, the message "Hold for <br /> Proposal Opening", and the deadline for proposal submission. NJPA cannot be responsible for <br /> late receipt of proposals. Proposals received by the correct deadline for proposal submission will <br /> be opened and the name of each Proposer and other appropriate information will be publicly read. <br /> 3.12 Corrections, erasures, and interlineations on a Proposer's Response must be initialed by the <br /> 10 of 47 <br />
