29 <br /> 10.5 Life Insurance <br /> <br />The City shall continue to offer to eligible employees the life insurance <br />program in effect on February 1, 1974. Effective September 1, 2011, the City <br />shall provide employees Basic Life Insurance coverage of $50,000, and <br />Accidental Death and Dismemberment (ADD) coverage of $50,000, with the City <br />paying 100% of the premium. <br /> <br />10.6 Long Term Disability Insurance <br /> <br />Effective with the first full pay period after the adoption by the City Council of <br />the Memorandum of Understanding between the POA and the City of Redwood <br />City, the City will reimburse $19.50twenty-four dollars and fifty cents ($24.50) <br />per month, per employee towards the cost of Long Term Disability. <br /> <br />In the event the cost of the current Long Term Disability plan increases during <br />the term of this Memorandum of Understanding, the City will reimburse the <br />actual premium cost up to a maximum of twenty five dollars ($25.00) per <br />month per employee. <br /> <br />10.7 Vision Care <br /> <br />The City will contract with Vision Service Plan (VSP) or comparable vision care <br />provider to provide vision care benefits for employees and their dependents. <br />The Vision Service Plan B provides for an exam every 12 months, lenses every <br />12 months if needed, and frames every two years if needed. There is no <br />deductible for employees, but a $20.00 per person deductible will apply to <br />dependents each time benefits are available and will be paid by the employee. <br />Effective January 1, 2016, the City’s contribution toward the plan shall be <br />ninety percent (90%) of the plan premium. <br /> <br /> <br />SECTION 11. PROBATION <br /> <br />11.1 New Employees <br /> <br />An employee shall serve a probationary period of eighteen (18) months. New <br />hire lateral Police Officers shall serve a probationary period of twelve (12) <br />months. Upon satisfactory completion of such probationary period, the <br />employee shall be appointed as a regular full-time employee of the City of <br />Redwood City. <br /> <br />New employees on probation who are off work for more than 30 work days shall <br />have their probationary period extended by the same amount of time as the <br />work absence. <br /> <br />During the first six (6) months of his or her probationary period, an employee <br />may be required to pass physical agility and endurance tests, written or oral <br />examinations, and other job related subjectstests to continue employment as a <br />probationary employee. During the last twelve (12) months of the an eighteen <br />(18) month probationary period, an employee may be required to pass <br />6.1.C. - Page 40
