21 <br /> <br /> <br />9.6.2.2 Within thirty (30) days of the beginning of the maternity <br />pregnancy leave, the employee shall submit to the <br />Personnel Officer the specific date she intends to begin the <br />leave, accompanied by her physician's written statement <br />attesting to the worker's ability to continue performing the <br />full schedule of duties and responsibilities. She shall <br />continue her regular active duty until the specific date <br />providing she is able to perform the full duties and <br />responsibilities of her position and furnishes additional <br />health statements from her physician upon reasonable <br />request. In the event the employee is unable to perform <br />the full duties and responsibilities of her position (as <br />mutually agreed upon by the Police Chief, the Personnel <br />Officer, and with input from the employee’s attending <br />physician), she shall be assigned to light duty until the <br />specific leave date, and shall continue to furnish additional <br />health statements from her physician upon reasonable <br />request. <br /> <br />9.6.2.3 Prior to the establishment of a specific date for return to <br />duty, the employee shall submit to the Personnel Officer a <br />notice of intention to return to duty, accompanied by her <br />physician's statement certifying that the employee is <br />medically qualified to assume regular duties and <br />responsibilities. <br /> <br />9.6.2.4 The Personnel Officer or his/her designee may designate <br />the specific beginning and ending dates within the pay <br />period requested by the employee to meet the needs of the <br />employee and the City. <br /> <br />9.6.3 The employee on leave shall be returned to an equivalent position <br />within her classification. <br /> <br />9.6.4 A maternity pregnancy leave, absent physical disability, is granted <br />without pay for the duration of the leave. The employee may elect to <br />continue medical and dental insurance coverage for up to one (1) year <br />during that leave at her own expense. <br /> <br />The combination of pregnancy leave and parental leave shall not <br />exceed one (1) year. <br /> <br />9.7 Leave for Pregnancy Disability <br /> <br />9.7.1 Employees who are working are entitled to use personal illness and <br />injury leave for disabilities caused or contributed to by pregnancy, <br />miscarriage, childbirth, and recovery therefrom on the same terms <br />and conditions governing leaves of absence for other illness or medical <br />disability, and in accordance with the City’s policy on Pregnancy <br />6.1.C. - Page 32
