18.6 Maternity Leave of Absence Without Pay <br />18.6.1 Maternity leave of absence without pay or benefits shall be granted upon <br />request to non -disabled probationary and regular female employees for that <br />period of time requested up to one (1) year. <br />18.6.2 Maternity leave shall be granted when the following conditions have been <br />met: <br />18.6.2.1 The employee shall notify her immediate supervisor in writing <br />accompanied by her physician's certificate of pregnancy as soon as <br />possible after pregnancy has definitely been determined, but no <br />later than ninety (90) days prior to the tentative date on which the <br />leave is to begin. Such notice shall include the tentative dates on <br />which the leave shall begin and end. <br />18.6.2.2 Within thirty (30) days of the beginning of the maternity leave, <br />the employee shall submit to the Human Resources Director the <br />specific date she intends to begin the leave, accompanied by her <br />physician's written statement attesting to the employee's ability to <br />continue performing the full schedule of her duties and <br />responsibilities. She shall continue on active duty until the specific <br />date providing she performs the full duties and responsibilities of <br />her position and furnishes additional health statements from her <br />physician upon reasonable request. <br />18.6.2.3 Prior to the establishment of a specific date for return to duty, <br />the employee shall submit to the Human Resources Director a <br />notice of intention to return to duty, accompanied by her <br />physician's statement certifying that the employee is medically <br />qualified to assume regular duties and responsibilities. <br />18.6.2.4 The Human Resources Director or his/her designee may designate <br />the specific beginning and ending dates within the pay period <br />requested by the employee to meet the needs of the employee and <br />the City. <br />18.6.3 The employee on leave shall be returned to her original position or if that <br />position is not in existence, to an equivalent position within her classification. <br />18.6.4 A maternity leave, absent physical disability, is granted without pay for the <br />duration of leave. The employee may elect to continue medical and dental <br />insurance coverage for up to one (1) year during this leave at her own expense. <br />
