6.1.B. - Page 22 <br />All samples to be analyzed by the Contractor will be accompanied by a chain of custody (COC) form. <br />In the event COC information is in error or there is a question or clarification regarding information <br />on the COC, Contractor will contact PWS immediately and prior to proceeding with the analysis. <br />A completed copy of the COC, signed by Contractor, will accompany all sample reports. <br />E. Term <br />The term shall be for a two (2) year period from the effective date of the contract. The contract <br />may be renewed for up to two (2) consecutive, two (2) year terms after the initial contract period, <br />at the discretion and approval of the City. The City shall notify the Contractor in writing of the <br />intent to extend the contract. <br />F. UCMR Analvses <br />The City is required to collect samples under the Unregulated Contaminant Monitoring Rule Part 4 <br />(UCMR4). Samples collected for UCMR4 must be analyzed according to UCMR guidelines by an <br />USEPA approved laboratory. <br />G. Invoicing <br />Contractor will establish individual accounts or sub -accounts for PWS divisions and submit monthly <br />invoices to the appropriate division. The divisions are: <br />• Drinking Water <br />• Storm <br />• Recycled Water <br />• Wastewater <br />Invoices will also include the appropriate project number and description. Invoices will be mailed <br />to: Public Works Services Department, Attn: (Division Name), 1400 Broadway, Redwood City, CA <br />94063. A sample invoice will be included in the submitted bid. <br />Payment will be made within thirty (30) days of receipt of an accurate invoice. Payment will be <br />made no more frequently than monthly. <br />H. Redwood Citv Business license <br />The Successful Bidder must either possess a current, valid Redwood City business license or have <br />submitted a Redwood City business license application and fee at the time of contract award. <br />Business license information (application, ordinance, fees) can be found on the City's website at <br />httr)://www.redwoodcitv.orR/departments/administrative-services/finance/taxes- <br />assessments/business-licensing. <br />I.
